About The Position

Provides services and support to individuals to develop and enhance community and independent living skills. Instructs, assists, and monitors clients in accordance with service plans. Assists in providing a safe and therapeutic environment. Under general supervision, participates in the initial assessment, intervention, planning and re-evaluation of client’s needs based on behaviors. Leads the daily operations, purchasing, compliance and licensing of the food services program. Will help prep, cook, serve, and clean up after daily meals.

Requirements

  • Work requires the ability to read, write, and speak English and working knowledge of specific patient population served, e.g., children and adolescents or adults with mental disorders.
  • High school Diploma or equivalent required.
  • Work requires current, valid SERVSAFE certification upon hire or within 60 days of hire.
  • Ability to obtain additional certifications/trainings as may be required by certain programs (e.g., CPR/First Aid, Infection Control, Food Safety and Handling, and Crisis Intervention/Suicidal Protocol).
  • Work requires orientation period to obtain sufficient familiarity with behavioral rehabilitation methods, procedures and techniques, program policies and procedures, and specific patient population.
  • Requires valid state driver's license and must meet requirements as outlined in Sheppard Pratt's transportation policy. Depending on company vehicle availability, may be required to utilize personal vehicle for transportation purposes, including transporting clients as necessary. When access to a personal vehicle is required and proof of insurance is required. Certain programs and positions may require access to a vehicle during working hours for work related duties, including transporting clients as necessary. When access to a vehicle is required vehicle insurance must name the employee as an insured driver.
  • The ability to physically assist clients with daily tasks.
  • Must possess direct knowledge of or be willing to learn and implement the CACFP program.

Responsibilities

  • Works with clients to learn or regain skills for successful community living including independent living skills (shopping, cooking, cleaning, budgeting and transportation) and socialization (enhancing social and/or romantic relationships, recreational and leisure pursuits that contribute to community integration) utilizing deliberate and consistent skills training which typically includes staff demonstration, working side by side, facilitating groups, client practice/roleplaying and staff feedback, as well as prompting and cueing.
  • Participates in the Individual Rehabilitation Plan (IRP) process by drafting, re-framing and finalizing IRPs in collaboration with the client.
  • Monitors clients’ medications, ensuring compliance with medical regimen.
  • Coordinates comprehensive care to assigned clients.
  • Maintains accurate, up-to-date documentation of all services in a timely manner. In addition to contact notes, end-of-shift notes, and incident reports, is responsible for monthly progress notes, IRPs and utilization and documentation of appropriate assessment tools (DLA-20s), as directed by supervisor.
  • Assist in the daily food operations and as-needed catering operations. Will help ensure that meals are attractive, nutritionally complete, appealing and delivered fresh and hot. Will coordinate the preparation and delivery of off-site meals.
  • Helps ensure that food service documentation and licensing is accurate, complete and current. Will also ensure compliance with all Department of Labor and State Laws, regulations, guidelines, and directives. Will participate in all required food service trainings.
  • Ensure overall sanitation and Department of health regulations are met and exceeded. Ensures all CACFP documentation is completed accurately and timely.
  • Maintain proper cash control and end of day counts of food service program.
  • Supports clients in maintaining rules and basic program guidelines. With guidance from supervisor, provides crisis support/intervention and facilitates appropriate utilization of community support systems.
  • Provide transportation to clients in a personal vehicle and/or agency-owned vehicles including large passenger vehicles as assigned.
