About The Position

We are seeking an experienced Psychiatric Nurse Practitioner with a strong background in geriatric psychiatry to provide on-site behavioral health services within skilled nursing, rehabilitation, and long-term care facilities. Requirements

  • Board-certified Psychiatric Nurse Practitioner
  • Active, unrestricted state license
  • Current DEA registration
  • Minimum of 2+ years of experience managing psychiatric conditions in geriatric or long-term care populations
  • Demonstrated experience with dementia-related behavioral management
  • Strong clinical judgment in complex, medically fragile populations
  • Ability to work autonomously with minimal supervision

Nice To Haves

  • Extensive experience in skilled nursing or long-term care psychiatry
  • Proven track record reducing inappropriate antipsychotic utilization
  • Comfort managing high-acuity behavioral cases in non-hospital environments
  • Experience working within value-based or quality-driven care models

Responsibilities

  • Perform comprehensive psychiatric evaluations in post-acute and long-term care settings
  • Independently manage chronic psychiatric conditions including mood disorders, psychosis, anxiety disorders, and trauma-related disorders
  • Assess and treat Behavioral and Psychological Symptoms of Dementia (BPSD)
  • Optimize psychotropic regimens while minimizing polypharmacy and medication-related adverse events
  • Conduct capacity assessments and risk evaluations when appropriate
  • Lead behavioral health strategy within assigned facilities
  • Collaborate with medical directors, nursing leadership, social services, and rehabilitation teams
  • Provide staff education on behavioral management and psychotropic best practices
  • Ensure compliance with CMS regulations and state long-term care standards
  • Maintain timely, defensible, and regulation-compliant documentation
