Requirements
- Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP)
- Active and unrestricted New York State Nurse Practitioner license
- DEA registration or ability to obtain
- Demonstrated ability to work independently with sound clinical judgment
- Prior experience in geriatric psychiatry, long-term care, skilled nursing, or similar settings preferred
- Experience managing dementia-related behaviors, chronic psychiatric conditions, and complex medication regimens is highly desirable
- Strong interpersonal and communication skills
- Comfortable practicing in nursing home and long-term care environments
Responsibilities
- Conduct thorough psychiatric assessments for residents in nursing homes and long-term care settings
- Manage ongoing psychiatric care, including follow-up visits and medication oversight
- Identify and treat a range of behavioral health conditions commonly seen in the elderly population
- Initiate and adjust psychiatric medications based on clinical presentation and response to treatment
- Develop and implement individualized care strategies tailored to each resident’s needs
- Collaborate closely with interdisciplinary teams including nursing, medical providers, social services, and facility leadership
- Provide guidance to facility staff on behavioral management and psychiatric care approaches
- Maintain accurate, timely, and compliant clinical documentation
- Communicate effectively with residents, family members, and care teams to ensure continuity of care
- Contribute to overall quality and consistency of behavioral health services within assigned facilities
