We are actively hiring experienced and compassionate Psychiatric Mental Health Nurse Practitioners (PMHNPs) to provide behavioral health services within long-term care, skilled nursing, and post-acute care environments across Tompkins County and nearby regions. Requirements

  • Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP)
  • Active and unrestricted New York State Nurse Practitioner license
  • DEA registration or ability to obtain
  • Demonstrated ability to work independently with sound clinical judgment
  • Prior experience in geriatric psychiatry, long-term care, skilled nursing, or similar settings preferred
  • Experience managing dementia-related behaviors, chronic psychiatric conditions, and complex medication regimens is highly desirable
  • Strong interpersonal and communication skills
  • Comfortable practicing in nursing home and long-term care environments

Responsibilities

  • Conduct thorough psychiatric assessments for residents in nursing homes and long-term care settings
  • Manage ongoing psychiatric care, including follow-up visits and medication oversight
  • Identify and treat a range of behavioral health conditions commonly seen in the elderly population
  • Initiate and adjust psychiatric medications based on clinical presentation and response to treatment
  • Develop and implement individualized care strategies tailored to each resident’s needs
  • Collaborate closely with interdisciplinary teams including nursing, medical providers, social services, and facility leadership
  • Provide guidance to facility staff on behavioral management and psychiatric care approaches
  • Maintain accurate, timely, and compliant clinical documentation
  • Communicate effectively with residents, family members, and care teams to ensure continuity of care
  • Contribute to overall quality and consistency of behavioral health services within assigned facilities
