Who You Are You are a dedicated and compassionate Psychiatric Mental Health Nurse Practitioner who thrives in dynamic, community-based environments. What You Will Do As the Psychiatric Nurse Practitioner for the Lazarus Project, you will provide high quality, trauma informed psychiatric care across community outreach and residential settings. You will work with individuals experiencing chronic homelessness, severe mental illness, substance use disorders, and co-occurring conditions, meeting them where they are to support stabilization and engagement in treatment. By collaborating closely with the Medical Director, case managers, outreach teams, and residential staff, you will help ensure continuity of care and improved outcomes for some of the community’s most vulnerable individuals. Community Outreach Psychiatry Psychiatric evaluations: Conduct assessments and follow up visits directly in community settings including encampments and outreach zones. Medication management: Provide medication monitoring, adjustments, and education in field environments. Crisis intervention: Deliver brief therapeutic interventions and assess individuals during psychiatric or behavioral health crises. Outreach engagement : Participate in early morning, evening, weekend, and special outreach operations to reach individuals who are otherwise difficult to engage. Multidisciplinary collaboration : Work alongside outreach teams to support stabilization and connection to care. Residential Treatment Center Responsibilities Resident evaluations: Provide psychiatric assessments for individuals in the Camillus House Residential Treatment Center. Medication follow up: Manage psychiatric medications for residents and ensure continuity of care. Urgent clinical support : Respond to time sensitive psychiatric needs within residential programs. Medication nurse supervision: Guide to the Medication Nurse to ensure proper medication administration, documentation, and adherence to protocols. Disability Evaluations and Documentation Disability assessments: Conduct psychiatric disability evaluations when required. Clinical documentation: Complete comprehensive documentation to support disability claims or coordinated care efforts. Care communication: Share evaluation findings with the supervising psychiatrist and care teams to support treatment planning. Collaboration With the Medical Director Case consultation : Review complex cases and diagnostic considerations with the Medical Director. Clinical participation: Attend clinical meetings, case staffing discussions, and quality improvement activities. Standards compliance: Ensure all care aligns with clinical best practices, Camillus House standards, and regulatory requirements. Cross Program Support Program evaluations: Provide psychiatric evaluations to clients across additional Camillus House programs such as day centers, behavioral health programs, and supportive housing. Care transitions: Assist in ensuring medication adherence and continuity of care when clients move between programs. Compliance, Documentation, and Professional Standards Timely documentation: Complete all required documentation according to agency and regulatory standards. Record accuracy: Maintain clear and accurate treatment plans, medication records, and clinical notes. Ethical practice: Uphold HIPAA compliance, ethical care principles, and trauma informed standards in all interactions.
