Review our Top 10 Tips for Success when Applying to State of Texas Jobs.   Under the supervision of the Regional Manager, Specialized Health/Social Work Services, is responsible for provider relations and recruitment in Region 4/5N. Duties include, but not limited to, provider enrollment and support, education, training assistance and coordination of special projects with Texas Health Steps Lead. Conducts presentations and communicates program services and/or changes to consumers/public in a positive and accurate manner. Maintains the integrity and accuracy of the Regional provider database. Acts as representative for Childhood Lead Poisoning Prevention Program and assures coordinated appropriate follow-up on children with high blood levels of lead. Collaborates with agency/program staff and actively participates in Community Resource Coordination Groups (CRCG), community organizations and coalitions. Completes administrative duties, monthly reports, special assignments and work activities in a professional and timely manner. Works collaboratively with public health staff in the event of a regional or state disaster. Works under minimum supervision with considerable latitude for the use of initiative and independent judgment. Maintains confidentiality. Attends to other assignments as directed by the Manager of Specialized Health/Social Work Services, and/or designee.  As needed, participates in disaster response including Continuity of Office Operations activities.

Requirements

  • Knowledge of public/private health care programs, policies and procedures, including CSHCN, CHIP, and Medicaid/THSteps Program.
  • Knowledge of social service programs, policies and procedures.
  • Knowledge of corrective action planning, monitoring and evaluating.
  • Knowledge of DSHS contract regulations; guidelines.
  • Knowledge of program planning and implementation and of policies and regulations governing the THSteps Program
  • Knowledge of quality assurance and technical assistance policies and procedures.
  • Knowledge of community resources on a local, state and national level.
  • Knowledge of the cultural and socioeconomic issues related to health and disabilities.
  • Skills in program development, management, monitoring and operations.
  • Skills in organizing workloads and setting priorities to carry out assigned tasks in a timely manner.
  • Skills in service delivery development, negotiations and management.
  • Skills in using computer software programs in word processing, spreadsheets, databases, electronic mail and Internet.
  • Ability to plan and direct the work of others and to develop and administer program, training and workshops.
  • Ability to establish and maintain effective working relationships with professional, civic, and lay groups.
  • Ability to develop, implement, and administer components of health care programs.
  • Ability to define problems, make appropriate decisions, analyze data and reports for administrative use.
  • Ability to work effectively with others in a multi-disciplinary setting.
  • Ability to communicate effectively, orally and in writing.
  • Ability to cooperate and collaborate with diverse professional interests, and with Federal, State, and local officials and the general public.
  • Ability to follow the National Association of Social Workers Ethical Standards.
  • Bachelors degree in Social Work or related humanities degree required.

Responsibilities

  • Maintains provider base adequate to offer target population access to a system of child health services.
  • Coordinates, plans and implements special projects designed to improve program utilization and barrier reduction and Corrective Action Plans as outlined by the THSteps program staff.
  • Maintains provider base data and documents activities with providers for required reporting by required deadlines.
  • Collaborates with agency staff, communities and agencies in delivery of services and program utilization by shared, eligible population.
  • Provides staff development, and consultation on principles and practices of community organization.
  • Represents the agency as assigned on regional, inter-agency and community groups, including Community Resource Coordination Groups (CRCG).
  • Attends community events related to program needs.
  • Provides presentations and information to stakeholders and other community members as needed.
  • Participates and collaborates with local, state and federal social service entities and others to promote efforts to service families with children who have health risks, conditions and special health care needs.
  • Actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.
