About The Position

The PROS Clinician – Senior is a member of the PROS clinical team whose primary role is to provide assessment, recovery planning and clinical treatment to PROS participants. The PROS Clinician – Senior assists participants with goal acquisition, intensive relapse prevention (and other EBP), management of clinical symptoms and elimination of barriers to recovery. STATUS: FULL TIME LOCATION: GMHC PROS DEPARTMENT: BH SCHEDULE: 40 HOURS, MON-FRI 8-4:30

Requirements

  • Must meet the definition of “Professional Staff” as defined by the Office or Mental Health regulations part 512.4 (z)
  • One year of experience preferred.
  • Strong analytical, computer, and communication skills.
  • If hired on or after January 1, 2011, Master’s Degree in Social Services field required

Responsibilities

  • Provides initial and ongoing assessment, counseling and clinical intervention to a designated caseload.
  • Provides PROS participants support for achievement in employment and other recovery goal acquisitions.
  • Provides individual, group, and family psychoeducation, and monitors patient response to psychotropic medications.
  • Provides Engagement and Outreach when necessary.
  • Focuses on skill building, goal acquisition, assisting participants with recovery.
  • Develop/facilitate class curriculum for skill building classes.
  • Participates functionally as a member of an interdisciplinary team in coordination and delivery of care.
  • Demonstrates knowledge of and provides linkage with community and natural supports.
  • Communicates with relevant internal and external care providers to complete ongoing assessment to coordinate care.
  • Participates in and incorporate professional supervision and ongoing education toward quality care, especially attends to training in Evidence Based Practices in PROS program (i.e., COD, Family Psychoeducation).
  • Completes all required documentation in a timely and comprehensive manner.
  • Maintains a full caseload, meeting productivity requirements and seeks to keep the incidence of failed appointments to a minimum.
  • Uses recovery and person centered approaches, including person first language when working with consumers and families.
  • Provides ongoing support for participants for goal acquisition (i.e., employment).
  • Individual Recovery Plans – Incorporate individual strengths, aspirations, barriers from assessments and reflect participants’’ full input.
  • Participants on assigned caseload have clearly defined recovery goals which are stated in their own words.
  • Demonstrates safe driving practices in accordance with Rochester Regional Health’s driver policy.
  • Participates fully in Quality Improvement activities within the PROS program and department.
  • Collaborates and communicates with primary care providers and multidisciplinary team as appropriate and documents contacts in participant’s record.
