About The Position

The Program Supervisor of Street Outreach provides housing-focused, trauma-informed, and person-centered outreach and case management services to individuals experiencing unsheltered homelessness. In addition to maintaining an outreach caseload, the Lead Street Outreach Case Manager serves as the team leader for MHA’s Street Outreach program, providing coordination, guidance, and support to outreach staff and acting as a primary liaison with community partners. The Program Supervisor engages individuals living in encampments, vehicles, abandoned buildings, wooded areas, and other unsheltered locations. Services are grounded in consistency, persistence, flexibility, and respect for participant choice. They supports participants in connecting to Coordinated Entry, completing housing-related documentation, and maintaining engagement until homelessness is resolved. The Program Supervisor will spend most of their time in the field and community and will regularly travel to MHA offices as needed.

Requirements

  • Ability to work independently and as part of a team.
  • Strong organizational, communication, and interpersonal skills.
  • Capacity to adapt to the evolving needs of participants.
  • Must be at least 21 years of age and able to obtain/maintain CPR and First Aid certifications.
  • High school diploma or equivalent, required.
  • Minimum of two years of relevant experience working with individuals experiencing homelessness, mental health challenges, or substance use disorders, required.

Responsibilities

  • Conduct consistent, trauma-informed street outreach to unsheltered individuals and build trusting, person-centered relationships over time.
  • Assess participants’ housing needs, preferences, and barriers, and collaborate on individualized, housing-focused plans at a pace that respects readiness.
  • Facilitate access to the CoC Coordinated Entry system, including completing assessments and supporting prioritization and case conferencing processes.
  • Support participants in becoming document-ready for housing placement (ID, income verification, benefits, required documentation).
  • Maintain engagement with participants until homelessness is resolved, including during transitions into permanent housing.
  • Coordinate with internal MHA programs and external housing partners to ensure timely housing placements and effective service delivery.
  • Serve as a primary point of contact for community partners (e.g., law enforcement, crisis teams, CoC representatives) and coordinate outreach priorities based on referrals.
  • Provide leadership and guidance to Street Outreach Case Managers, including mentorship, safety planning, and field coordination.
  • Connect participants to community resources, public benefits, healthcare, shelter, warming centers, transportation, and treatment services as needed.
  • Ensure accurate and timely documentation in HMIS and MHA’s electronic health record, monitor outreach data and outcomes, maintain compliance with policies, and actively participate in supervision, trainings, and team meetings.
