About The Position

The Program Specialist at Lehigh Valley Companion Care Homes, LLC (LVCCH, LLC) plays a central role in ensuring the effective coordination, quality, and compliance of services provided to individuals with intellectual disabilities and autism. Responsible for the oversight of no more than 30 participants, the Program Specialist ensures that all Individual Support Plans (ISPs), program records, and service activities align with the organization’s mission and with ODP, DHS, and 55 Pa. Code Chapter 6400 regulatory requirements. This role encompasses leadership in staff supervision, documentation management, compliance monitoring, and program quality, including oversight of Lifetime Medicals, program books, financial ledgers, and appointment forms, ensuring the highest standard of care and accountability across all residential sites.

Requirements

  • Master's degree in special education, Psychology, Social Work, or a related human services field with at least one (1) year of experience working with individuals with intellectual disabilities or autism; or
  • Bachelor’s degree with two (2) years of direct experience; or
  • Associate’s degree/60 college credits with four (4) years of direct experience supporting individuals with intellectual disabilities or autism.
  • Strong working knowledge of ODP, 55 Pa. Code Chapter 6400, waiver compliance, and licensing standards.
  • Excellent leadership and supervisory skills, with the ability to manage multiple residential sites and staff teams.
  • Strong written and verbal communication abilities.
  • In-depth understanding of ISP processes, documentation requirements, and audit readiness.
  • Proficiency in Evolv, QuickMar, ADP Workforce Now, and Microsoft Office applications.
  • Detail-oriented, organized, and capable of managing complex schedules and regulatory tasks.
  • Compassionate, person-centered approach with strong cultural competence.
  • Demonstrated ability to problem-solve and adapt to evolving needs and priorities.

Responsibilities

  • Coordinate completion of assessments and ensure all required documentation is current for each individual.
  • Lead the development, implementation, and monitoring of Individual Support Plans (ISPs) in compliance with state regulations.
  • Conduct quarterly ISP reviews and submit quarterly reports within 14 days after the due date.
  • Facilitate person-centered care, ensuring individual preferences, strengths, and goals are reflected in all supports.
  • Promote community integration and family involvement through meaningful engagement activities.
  • Serve as the program’s Communication Specialist, maintaining ODP Communication Specialist certification.
  • Facilitate both on-site and virtual communication between individuals, families, and interdisciplinary teams.
  • Ensure that individuals’ communication preferences and needs are respected and supported through appropriate tools and training.
  • Act as liaison between families, care providers, and external agencies to ensure smooth information flow.
  • Supervise and manage House Managers, ensuring they meet performance expectations and uphold regulatory standards.
  • Approve and monitor House Manager timesheets to ensure accuracy and compliance with payroll procedures.
  • Participate in House Manager interviews and provide input during hiring decisions.
  • Conduct orientation and training for Direct Support Professionals (DSPs) on ISPs, communication, documentation, and daily support expectations.
  • Provide ongoing coaching, performance reviews, and corrective feedback as needed.
  • Promote a culture of accountability, teamwork, and respect within the program.
  • Maintain comprehensive program books for all individuals, ensuring each includes: Updated appointment forms and medical documentation; Current activity calendars reflecting engagement and community participation; Accurate cash ledgers and individual receipts for all personal funds and expenditures; Current copies of ISPs, quarterly reports, incident reports, and communication logs.
  • Audit all program books monthly to ensure regulatory and internal compliance.
  • Ensure that documentation is easily accessible for county, state, and ODP audits.
  • Collaborate with the administrative and QA teams to correct deficiencies identified during reviews.
  • Conduct weekly house visits to ensure compliance with ODP, DHS, and licensing standards.
  • Perform health and safety inspections, document findings, and ensure prompt resolution of issues.
  • Maintain compliance with all Lifetime Medicals, ensuring medical summaries, assessments, and histories are up to date.
  • Prepare and submit documentation for county and state audits, ensuring accuracy and timeliness.
  • Ensure the program meets all performance-based contracting (PBC) requirements and outcome standards.
  • Complete and review QuickMar and ADP Workforce Now audits as needed for quality assurance.
  • Manage petty cash vouchers, per diem cards, and Cash on Hand (COH) balances across all assigned homes.
  • Verify accuracy and completion of individual cash ledgers and receipt documentation for all purchases and withdrawals.
  • Approve and submit House Manager timesheets following verification of staff attendance and hours worked.
  • Oversee fiscal accuracy and maintain transparent records for audits and internal reviews.
  • Support the CFO and administrative team in ensuring financial compliance and budget alignment for program sites.
  • Ensure residents have opportunities for meaningful community participation and inclusion consistent with their ISPs.
  • Oversee Assessment of Service Needs (ASA) and ensure service delivery reflects identified outcomes.
  • Strengthen partnerships with families, community partners, and interdisciplinary teams to improve quality of life and service outcomes.
  • Maintain accurate records in HCSIS, EIM, and other internal systems.
  • Ensure all incidents, ISP updates, audits, and quarterly reports are completed and submitted within regulatory timelines.
  • Conduct orientation and training for Direct Support Professionals (DSPs) on ISPs.
  • Collaborate with the Director of Quality Assurance during internal and external audits.
  • Participate in ongoing monitoring to ensure compliance with waiver, licensing, and ODP performance standards.
  • Track and report key metrics related to program compliance, service quality, and individual outcomes.
