About The Position

The Program Specialist at iHomecare Solutions is responsible for managing the care and casework for up to 30 individuals with intellectual disabilities and/or autism. This role serves as an immediate supervisor for Direct Support Professionals (DSPs) and plays a vital role in developing and overseeing individualized support plans (ISP) to ensure the health, safety, and well-being of residents. The position requires regular travel to residential homes and participation in non-traditional hours, including on-call duties.

Requirements

  • Master's degree and 1 year of experience with individuals with intellectual disabilities or autism, OR Bachelor's degree and 2 years of experience, OR Associate degree and 4 years of experience.
  • CPR and First Aid certification (or willingness to obtain).
  • Must have access to a reliable vehicle for regular travel.
  • Willingness to work non-traditional hours and cover DSP shifts as needed.

Responsibilities

  • Supervise DSP staff in residential homes, including training and, if necessary, issuing disciplinary actions.
  • Provide guidance and feedback on staff performance related to resident care and ISP goals.
  • Lead and actively participate in the creation and maintenance of ISPs to meet individual needs.
  • Conduct team meetings, write measurable goals, and monitor progress toward objectives.
  • Coordinate timely completion of assessments and documents required for ISP review and revisions.
  • Complete and submit quarterly reports based on staff documentation and progress toward individual goals.
  • Ensure records are updated, accurate, and compliant with regulatory requirements.
  • Maintain regular and effective communication with family members, staff, county representatives, and healthcare providers.
  • Relay updates related to ISP changes, health concerns, or behavioral needs to relevant team members and staff.
  • Collaborate with House Leads to ensure appropriate staffing levels and oversee onboarding activities for new hires.
  • Provide modeling and support to DSPs in implementing ISPs, ensuring individualized choice and resident involvement.
  • Ensure the health, safety, and security of individuals and promote their participation in community activities.
  • Monitor the use of adaptive equipment and provide relevant training to staff.
  • Oversee compliance with all regulatory standards, including audits, QA&I processes, and licensing requirements.
  • Assist with unannounced visits and ensure all assessments, appointments, and follow-ups are conducted as required.
  • Oversee the upkeep of physical sites, ensuring cleanliness, safety, and functionality.
  • Ensure adaptive equipment is present, well-maintained, and functional.
