About The Position

Responsible for the coordination of services, communication of service needs and documentation of service delivery for an identified caseload of individuals. Works collaboratively to ensure that services are delivered in accordance with each program participant’s Individual Support Plan (ISP); and that relevant agencies and regulatory guidelines are met. Serves as the agency lead of the Interdisciplinary Team.; Rectify service billing daily and weekly to ensure completion and accuracy.

Requirements

  • Bachelor’s or Associate degree required.
  • Strong written and verbal communication skills.
  • Ability to read and write in English.
  • Moderate mathematics skills required.
  • Proficient interpersonal relations and communication skills with parents, caregivers, residential providers, paraprofessionals and others.
  • Auditory and visual skills.
  • Ability to bend, stoop, sit, stand, reach, and lift items weighing 50 pounds or less.
  • Satisfactory completion of pre-employment physical exam and drug test as required.
  • Completion and clearance of a criminal background check.
  • A valid driver’s license with a clear three-year driving record.
  • Computer skills including familiarity with Microsoft Word and Excel.
  • Ability to perform the physical duties required of Direct Support Professionals.
  • Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in providing various care needs; and operating a vehicle.
  • The employee must be able to regularly lift and/or move up to 40 pounds and occasionally must lift and/or move up to 50 pounds, including transferring of client.
  • Bachelor’s degree and two years’ experience working with individuals with Intellectual Disabilities or an associate’s degree and four years’ experience working with individuals with Intellectual Disabilities

Responsibilities

  • Responsible for the coordination of services, communication of service needs and documentation of service delivery for an identified caseload of individuals.
  • Works collaboratively to ensure that services are delivered in accordance with each program participant’s Individual Support Plan (ISP); and that relevant agencies and regulatory guidelines are met.
  • Serves as the agency lead of the Interdisciplinary Team.
  • Rectify service billing daily and weekly to ensure completion and accuracy.
  • Supports the Mission and Values of Indian Creek Foundation and the Day Service program.
  • Is knowledgeable of Individual Rights and ensures an atmosphere which allows for the privacy, dignity, and well-being of all individuals served in a safe, secure environment.
  • Supports, cooperates with, and implements specific procedures and programs for:
  • Safety, including universal precautions and safe work practices, established fire/safety plans, report and/or correct unsafe working conditions, equipment repair and maintenance needs.
  • Maintains confidentiality of all data, including individuals served, employee and operations data.
  • Comply with all regulatory requirements and agency policies as required by Human Resources, licensing and funding agencies.
  • Demonstrate knowledge and compliance with HIPAA standards.
  • Conducts oneself in a professional manner when representing Indian Creek Foundation.
  • Functions as a member of the Team:
  • Cooperate and work together with all co-workers; plan and complete job duties with minimal direct supervision, including demonstrating appropriate judgment.
  • Use tactful and appropriate communications in all situations.
  • Report and follow up regarding reported complaints, problems and concerns with appropriate personnel.
  • Promote positive public relations with individuals served, family members, other team members and community members.
  • Complete requirements for training, acceptable attendance, dress codes including personal hygiene, and other work duties as assigned.
  • Complete all documentation to ensure compliance with all regulations and agency policies.
  • Exhibits behavior as set forth by the Code of Conduct in the performance of their duties
  • Is responsible for being well versed in the functions of a Program Specialist as defined by regulation; and for completing these functions for their assigned caseload.
  • Responsible for the coordination of age-appropriate services to individuals on their caseload, including but not limited to:
  • Develop and implement services for each person according to their ISP, including assistance, training and support for the acquisition, maintenance or improvement of functional skills, personal needs, communication and personal adjustment.
  • Support opportunities to individuals for participation in community life.
  • Review outcomes and action steps and make recommendations for changes when needed.
  • Train Direct Support Professionals (DSP) and other program staff on details of each person’s ISP; the health and safety needs of individuals; the implementation of outcomes and action steps for participants on their caseload according to regulations and agency guidelines.
  • Ensure staff training is entered accurately into the Credible System and sent to the training department weekly.
  • Assist with documenting the exact arrival / departure times of each program participant daily.
  • Adheres to regulatory requirements for communicating the results of the initial assessment and any revisions to the individual and the team.
  • Responsible for the coordination and completion of timely assessments for each person on their caseload.
  • Attends meetings for and participates in the ongoing development of the ISP, annual updates and revisions for individuals on their caseload.
  • Responsible for the review, implementation, monitoring and evaluation of the ISP/IPP for individuals on their caseload; and for communicating discrepancies to the Supports Coordinator and team.
  • Assist in assessing program referrals for eligibility and appropriateness for ICF Day Services; participate in admission meetings and program tours; facilitate admission transition; and familiarize policy and procedures to ensure caregiver and individuals understand participation and safety expectations.
  • Responsible for reviewing each person’s participation and writing progress reports for individuals on their caseload; signing and dating the documents; and for reporting changes to the Supports Coordinator or plan lead.
  • Responsible for reviewing the ISP quarterly and any revisions with the individual and for providing documentation of the review to the Supports Coordinator and the team.
  • Responsible for informing team members of the option to decline the ISP Review.
  • Coordinates and attends regular monitoring visits with the Supports Coordinator.
  • Tabulate and provide behavior documentation to team members as requested and regulated.
  • Assist the Program Specialist Supervisor with administrative tasks to support department operations as assigned; may also be assigned special projects.
  • In the absence of Program Specialist Supervisor, Specialist will assume program duties as assigned.
  • Serve on agency-wide committees as assigned or desired as a representative of Day Services department.
  • Perform tasks which are supportive in nature, and which are essential functions of the job. These may include providing medical assistance, behavioral management, or direct hands-on care to consumers.
  • Meet on-going requirements to maintain current training certifications, regulatory requirements, and assignments.
