PROGRAM SERVICES EVALUATOR 2 Job Location: OFFICE OF SUPPORT SERVICES 3003 N CENTRAL AVE PHOENIX, AZ  85012 Posting Details: SALARY: $16.5220 HRLY/$34,365.76 SALARY  GRADE: 15 CLOSING DATE: APRIL 6, 2026 Job Summary: Assists with processing requests for placement services made by field staff for the Department of Child Safety (DCS) clients, and assigns providers based on the service referral guides. This position works with DCS field staff as well as with community providers in regards to matching, ending, or extending service authorizations in the Guardian database. In addition, this position communicates directly with community providers in regards to availability for services.

  • Ability to understand and adhere to the Policies and Procedures of Guardian
  • Ability to understand and adhere to Current state, federal and district laws and policies regarding child welfare services
  • The ability to secure and maintain an Arizona Fingerprint Clearance Card; and the ability to secure and maintain clearance from the DCS Central Registry.
  • High school diploma or GED
  • All newly hired State employees are subject to and must successfully complete the Electronic Employment Eligibility Verification Program (E-Verify).

Nice To Haves

  • Two years of college or university training with a minimum of 21 semester hours in child development, rehabilitation, social services, counseling, psychology, sociology or other closely related field.

Responsibilities

  • Receives/processes Service Approvals for living arrangements and services including; Kinship Home Study, Courtesy Supervision. This also includes manages and responding to email inquires about Service Approvals for living arrangements and services and verifying information has been correctly entered on a Service Request so the Service Approval can be processed and matched to a Contracted Provider.
  • Creates Service Requests in Guardian database for living arrangements and services. Logs processed referrals as well as extensions assigned to providers daily. Ensures services are assigned per the service referral guide.
  • Maintains, enters, updates and tracks service authorizations and other Guardian data as requested by the supervisor.
  • Monitors and maintains the Resource Unit mailboxes. Ensures referrals are placed in the corresponding folders for assignment. Ensures logs and availability are updated daily.
  • Provides assistance either in person or by telephone or email to field staff or service providers.
  • Other duties as assigned as related to the position.
