About The Position

The Fortune Society is seeking an experienced NYS Health Home Care Management supervisor to manage the operations of our well-established, yet growing, care management department. This onsite position will have overall day-to-day responsibility for ensuring the connection of criminal legal system impacted adults, many of whom have complex health conditions, to a wide range of medical, mental, and social determinants of health services. The manager is a leader on the team, providing regular supervision to care managers and peer support specialists within the unit. This role requires attention to detail, ensuring each program’s policies and procedures are implemented and followed, and services provided are in alignment with both state regulations and Fortune’s mission. The role also includes working with the Behavioral Health division’s data analytics team to ensure the integrity of data reporting.

Requirements

  • Minimum of a master’s degree in a qualifying field or CASAC is required with at least 3 years of progressive experience in NYS Health Home Care Management;
  • Experience with Health Home Plus (inclusive of HARP), Serious Mental Illness (SMI) high needs individuals, including best practices and state/health home regulations;
  • Minimum of 2 years' experience in a supervisory capacity;
  • Ability to demonstrate strong written and verbal communication skills through reports, correspondence and presentations;
  • Strong organizational and interpersonal skills; ability to manage competing priorities within a growing unit;
  • Proficiency in MS Office, SharePoint, and Excel, as well as MAPP, ePaces, PSYCHES;
  • Ability to perform additional responsibilities as needed;

Responsibilities

  • Provide oversight of the Care Management Unit’s (CMU) care managers and peer support specialists, monitoring adherence to unit policies and procedures, caseload maintenance, and service provision;
  • Provide operational oversight of clinical and non-clinical staff who will be providing services to both Health Home (HH) and Health Home Plus (HH+) clients, monitoring adherence to NYS and OMH regulations;
  • Ensure all care managers are utilizing best practices to engage clients, including incorporation of trauma-informed, client-centered and strengths-based approaches;
  • Monitor the completion of assessments and the coordination of plan(s) of care to ensure adherence to NYS Health Home policies and procedures;
  • Screen participants for eligibility into Fortune’s CMU, including identification of HH+/high need eligibility and assign to care managers accordingly;
  • Monitor HARP assessments completed in the Medicaid Analytics Performance Portal (MAPP);
  • Manage care manager’s fieldwork, including days out of the office, location of service provision, and time and effort logs;
  • Establish and maintain internal and external partnerships with providers, establishing referral workflows for care managers to use with clients;
  • Support care managers in transitions of care service provision to ensure appropriate continuity of service delivery;
  • Ensure that individuals are being appropriately connected to supportive services such as, peer services, medication assistance/linkages to healthcare housing screenings and assistance with the HRA 2010e Supportive Housing applications;
  • Work closely with Fortune’s internal and external interdisciplinary care teams including the PCP, psychiatrist, therapist, residential service providers, DHS case workers, substance abuse treatment programs, Social Care Network (SCN) partners, and/or participants’ family members, etc., to determine appropriate service referrals;
  • Review/edit/approve the team’s encounter notes daily;
  • Monitor staff billing practices to ensure appropriate core services are entered and billed for each month according to acuity level;
  • Identify potential barriers to care and strategies for overcoming those barriers, including those relating to medication compliance;
  • Regularly liaise with Fortune’s housing screening team to identify potential placement opportunities for eligible individuals;
  • Attend and serve as the main point of contact at lead Health Home, NYS DOH and OMH meetings;
  • Complete all requisite agency and Health Home care management trainings;
  • Perform additional responsibilities as needed.
