About The Position

Hamilton-Madison House’s Supported Housing program is a NY State Office of Mental Health (OMH) licensed facility that provides permanent housing options for people who are seriously and persistently mentally ill (SPMI) to live independently in the community. The Program Manager will be responsible for the management and supervision of the program operations and staff. They will provide direct case management services; coordinate with internal and external medical, psychiatric and support service providers to ensure quality of care for all tenants. Work with property owners and vendors on leases, building maintenance, and help resolve tenant issues.

Requirements

  • Minimum BA/BS in Psychology, Social Work, or a related social service field and 3 years of relevant experience; Limited permit or licensed Master’s degree in Social Work, Psychology, Mental Health Counseling, Creative Arts Therapy, Occupational Therapy, and 1 year of relevant experience preferred
  • Driver’s license and good driving record required
  • Has S95 Certificate of Fitness or willing to obtain within 6 months of employment
  • Excellent interpersonal skills, good organizational skills, and managerial skills

Responsibilities

  • Oversee management and supervision of Supported Housing staff and volunteers
  • Interface and attend meetings with regulatory and licensing agencies such the OMH, Human Resources Administration (HR)a, Association for Community Living (ACL), and Center for Urban Community Services (CUCS), etc
  • Complete all necessary regulatory reports such as HRA-CAPS, incident management, etc when required
  • Provide case management services and other programming activities to program participants
  • Make client and home visits at a minimum of once a week, conduct on and offsite safety checks as needed
  • Work and coordinate with vendors and contractors to resolve maintenance or tenant issues
  • Work with clients to create Individual Service Plan (ISP) to increase independent living skills, self-sufficiency, income and residential stability
  • Meet with real estate agencies & landlords to initiate/renew lease agreements; pay monthly fees
  • Collect rent and utilities payment monthly from participants
  • Assist tenants in securing appropriate supportive services including mental health, primary care, substance use support, health home care coordination, home health aide, etc.
  • Respond to program participants needs such as obtaining forms, completing paperwork, contacting referrals, writing correspondence, making phone calls and resolving disputes, escort to medical appointment if needed
  • Assist in after hour consultation as needed, identify and provide intervention for potential crisis situations
  • Work closely with the staff of the state psychiatric centers on facilitating a smooth transition from the centers to a supported housing unit
  • Work with direct reports to set performance targets, complete annual appraisals; approve weekly timesheets and time off requests
  • Ensure program and tenant files are kept in compliance with HMH and OMH standards
