About The Position

The Program Manager is responsible to oversee, manage, coordinate and further develop healthcare related programs to ensure they benefit one another and meet business goals. This position leads programs and partnerships that address healthcare services will be the primary contact with program funders and be involved in program operations and possible development of new and similar programs. The Program Manager is responsible for ensuring goals and objectives are met as required.

Requirements

  • Bachelor’s degree in social work, human services or public health
  • Two or more years in social services or related field
  • Knowledge of community resources, healthcare and/or public health
  • Strong communication skills
  • Ability to generate, analyze and report date to drive improvement
  • Results oriented with the ability to prioritize and manage multiple programs simultaneously
  • Knowledge of protocols, procedures, regulations and standards
  • Ability to make effective administrative/procedural decisions and recommendations
  • Skill in the use of computers with a Windows-based operating environment
  • Maintains courtesy and diplomacy when interacting with clients
  • Able to work effectively at all levels in a collaborative team environment

Responsibilities

  • Implement and manage the assigned programs, program goals and metrics, monitoring progress, and managing relationships.
  • Provide comprehensive and actionable feedback to program partners to support program improvements
  • Collaborate with the clinical team to develop health promotion programs and interventions
  • Utilize statistical data to identify key health trends and priorities
  • Develop comprehensive program strategies that align with community needs and organizational goals.
  • Set clear objectives and KPI’s (Key Performance Indicators) for program success, considering statistical benchmarks
  • Collaborate with the performance improvement coordinator to collect, analyze and interpret health data to identify areas of improvement and measure program outcomes
  • Use statistical tools and models to track progress and make data driven decisions
  • Build and maintain relationships with community partners, healthcare providers, and managed care organizations
  • Collaborate with local organizations to leverage resources and expertise
  • Oversee the execution of health promotion initiatives, ensuring they are on schedule and within budget
  • Monitor the quality and effectiveness of programs and make necessary adjustments
  • Conduct regular program evaluations, using statistical analysis and patient satisfaction to assess impact and identify areas for improvement
  • Prepare reports and presentations for internal and external stakeholders
  • Engage in budget management as defined and directed by the Chief Medical Officer
  • Working with other departments to develop budgets and plans for the programs
  • Evaluating and assessing programs strengths and weaknesses
  • Maintain regular contact with funding organizations and report on program activities
  • Participate in the development, implement and regularly review and update relevant policies and procedures for the programs.
  • Ensure effective systems to track progress, and regularly access program components, so as to measure successes that can be effectively communicated to staff and funders.
  • Devising new programs that support the organizations objectives
  • Prepare and submit reports as required.
