About The Position

St. Patrick Center provides opportunities for self-sufficiency and dignity to people who are homeless or at risk of becoming homeless. Individuals and families build permanent, positive change in their lives through safe and affordable housing, sound mental and physical health, and employment and financial stability. Working collaboratively with individuals, businesses, government and other service providers, St. Patrick Center will end chronic homelessness in the St. Louis region through Housing First, an evidence-based practice that tells us to remove barriers, do intake and assessment early, move people into housing and offer them support to achieve their goals. Job SummarySt. Patrick Center, a ministry of Catholic Charities, is hiring a Program Manager to lead our Assertive Community Team (ACT). This pivotal role requires a dynamic leader who can effectively deliver treatment, rehabilitation, advocacy, and case management services tailored to the unique needs of our clients. As the Program Manager, you will be responsible for enhancing team performance while ensuring that our clients receive seamless and effective care. This position provides continuous support and guidance to clients and the ACT Team. We are looking for a professional with a strong understanding of principles and practices within the unhoused community. The core working hours are Monday through Friday, 8:00 am - 4:30 pm.Overview and Responsibilities

Requirements

  • Master’s degree in counseling/social work fields or related area required.
  • LMSW, LCSW, PLPC or LPC preferred.
  • Minimum of 2 years’ experience in supervising a team or relevant professional experience.
  • Driver license (MO residents require Class E), personal vehicle and auto insurance.
  • Drug test to be completed within 30 days of hire.
  • Knowledge of mental illness and chemical dependency.
  • Knowledge of Housing First and Trauma Informed Care.
  • Experience in counseling and case management.
  • Ability to lead, motivate team, and self-motivate.
  • Excellent communication skills, both verbal and written.
  • Counseling and crisis intervention expertise.
  • Respect for clients and coworkers and sensitive to cultural diversity.
  • Ability to work within a team and in a fast-paced environment.
  • Proficient computer and administrative skills.
  • Flexibility with daily schedule and ability to rotate on-call schedule.

Responsibilities

  • Direct the day-to-day clinical operations of the ACT team including scheduling coworker work hours to assure appropriate coverage; lead the daily and weekly organizational staff meetings, continuously evaluating the status of program clients.
  • Participate in recruitment, interviewing, hiring and work assignments, including development and implementation of coworker onboarding and annual performance plans.
  • Participate in referral and screening meetings with grant partners.
  • Conduct comprehensive, ongoing assessments of program clients’ mental illness, physical health, housing status and other relevant needs.
  • Provide direct clinical services to clients on an individual and group basis in the office and in community settings, making appropriate changes in treatment plans to ensure the client’s ability to cope with internal and external stresses.
  • Work with the service line leadership team on maintaining a culture of respect, empowerment, self-care, commitment to cultural competency, diversity and equity.
  • Refer clients to appropriate community agencies, assisting clients when needed.
  • Act as liaison and consult with community agencies and program clients to maintain coordination in the treatment process.
  • Collaborate with ACT team coworkers during daily and weekly staff meetings.
  • Facilitate clients’ treatment planning with an emphasis on facilitating personal growth and development towards recovery and wellness.
  • Provide on-call crisis intervention in coordination with ACT team coworkers, covering nighttime hours and weekends.
  • Maintain service line client tracking system and client files, documenting client progress to maintain a permanent record of client activity.
  • Respect and adhere to all ministry policies regarding work hours, building security and all written personnel standards.
  • Maintain good standing relationship with BJCBH.
  • Work with coworkers to maintain contracted 24-hour documentation into CaseWorthy HMIS system and BJCBH My Avatar.
  • Ensure the care model by interacting with all other service line programs.
  • Collaborate with BJC Behavioral Health and all Community Mental Health Centers.
  • Attend and participate in all appropriate service line meetings.
  • Other duties as assigned.
