About The Position

Upham’s Community Care, as a proud recipient of the Live, Long & Well grant from the Atrius Health Equity Foundation, seeks a dynamic individual ready to drive systemic change and advance community well-being. Upham’s Community Care commits to transforming health and economic opportunity in Boston neighborhoods. The Program Manager (PM) for the Together We Thrive (TWT) Collaborative oversees the rollout of a multi-organizational program, aligning health with economic opportunity. Reporting to Chief Sustainability Officer, the PM works with the Community Engagement Services, social services, clinical teams, and administrative staff to integrate cardiometabolic health and economic mobility strategies into everyday clinical workflows. This role leverages technology, strengthens collaboration, and builds capacity so the work remains relevant, measurable, and sustainable. Through strategic planning, community engagement, and partnership development, the PM helps create lasting change for the communities we serve.

Requirements

• Bachelor’s degree in Public Health, Social Work, Health Administration, or a related field (Master’s preferred).
• 3-5 years of experience in program management, community health, or multi-partner initiatives.
• Experience with grant writing, reporting, and funder deliverables.
• Knowledge of PHI and confidentiality.
• Strong computer skills and knowledge of Microsoft Office, Excel, Power Point, Outlook, and virtual platforms.
• Ability to analyze and interpret data to assess needs and evaluate engagement efforts.
• Experience in establishing partnerships with community agencies.
• Strong verbal and written communication skills, with the ability to build rapport with a variety of individuals and groups.
• Ability to manage multiple projects, prioritize tasks, meet deadlines, and work well with a collaborative team.
• Knowledge of social determinants of health, specifically cardiometabolic health and its intersection with economic mobility.
• Knowledge of Boston’s nonprofit landscape.
• Regular presence for community events, staff training, and committee meetings.
• Primarily on-site with flexibility for remote work as needed.
• Valid driver’s license and personal vehicle required.

Nice To Haves

• Familiarity with UCC neighborhoods and agencies ideal.
• Bilingual language skills are a plus.

Responsibilities

• Coalition Management (20%): Lead the rollout of the TWT Collaborative across UCC and partner organizations to ensure active participation beyond the duration of the grant period. Chair the TWT Steering Committee to deliver training resources and build curriculum aligned with grant objectives. Serve as primary communicator across all mediums, including social media, community events and partner meetings. Stay current on best practices and represent Collaborative in network and learning opportunities to strengthen local impact and trust.
• Community Outreach & Engagement (50%): Develop protocols that make program delivery accessible, efficient and sustainable. Coordinate and lead a minimum of 8-12 community outreach events monthly that connect residents with essential resources. Champion initiatives and policies that remove barriers to care, create pathways for economic opportunity, and support relevant and responsive practices. Collaborate regularly with teams to weave health and economic mobility strategies into workflows.
• Partnership Building & Expansion (20%): Build relationships to build visibility and trust in the community to expand the reach and impact of the program. Design and implement strategies to raise awareness of the TWT partners and local communities to increase partner engagement by 10-20% annually.
• Impact Tracking, Reporting, and Administration (10%): Conduct a community advisory group to identify service gaps and guide priorities. Use technology and data systems (data dash boards, grant management system tools) to streamline operations and reporting for efficiency. Present program workflows and grant award details to leadership teams across core organizations. Support grant writing and reporting to meet funder requirements.
