About The Position

MHP is hiring a Program Manager to provide overall service coordination of a Targeted Case Management team that provides flexible, community based peer support to individuals with co-occurring disorders and individuals experiencing homelessness. These services are designed to promote the stability, recovery and community integration of individuals, who have mental health conditions, may use substances and who may also have physical health concerns. Program Managers provide overall coordination of services for their assigned programs and mentor and develop their staff. Program Managers are also responsible for maintaining oversight to ensure that participants' self-identified goals for recovery; rehabilitation and community integration are supported by appropriate services and best practices. The Program Manager of Targeted Case Management is a Full Time role located at 7200 Chesnut St, Upper Darby, PA 19082. The successful candidate will work 40 hours per week, Monday - Friday, 8:30 AM - 5:00 PM.

Requirements

  • Master's Degree in a human services field required.
  • 2 years of experience in mental health direct care experience.
  • 3 years' experience in management/supervision required (including managing functions, processes, and/or people in a work, internship or volunteer setting).
  • Knowledge of clinical theory and practice, community resources and systems, financial/social resources, and experience in interviewing, assessment and implementation.
  • Must possess basic computer skills to perform job duties including desktop computing, e-mail, timesheet management, electronic health records, internet research, etc. using relevant software.
  • Must be able to work under pressure and meet deadlines and billable unit requirements, while maintaining a positive attitude and providing exemplary customer service.
  • Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices.
  • Ability to understand and effectively manage a program budget.
  • Strong skill sets in oral and written communication, cultural competence, interpersonal relationships and teamwork, attention to detail, customer service, time management, autonomy, problem solving and organization.
  • Must possess a valid driver's license.
  • Must have access to a car to use for work on a daily basis. Must have automobile insurance coverage at the state requirement levels and current state registration.

Responsibilities

  • Leads the overall service coordination consistent with MHP & regulatory guidelines; ensures adherence with all accreditation, certification and regulatory standards if applicable; establishes and implements operational procedures and monitors participant visits.
  • Hires, trains, evaluates and develops staff in support of MHP and Recovery core policies, practices and principles; addresses and monitors the specific goals and assignments of each staff member by facilitating and documenting individual supervision every other week and morning team meetings daily. Manages program staff schedule.
  • Cultivates and maintains working relationships with outside agencies, i.e. behavioral health organizations, healthcare providers, and other community organizations; attends professional meetings as required; engages with the community to increase visibility in the community and generate referrals and partnerships.
  • Reviews and approves employees' electronic health record entries; ensures participant satisfaction through service verifications, auditing and monitoring records, shadowing, etc.
  • Assists participants with mental health and substance abuse services, acquiring stable housing, and other participant-identified goals in accordance with their support plan; ensures continuity of care.
  • Tracks revenue from mobile services and incorporates individual production reports into staff supervision sessions to support staff in achieving billable unit quotas.
  • Defuses critical situations and provides safety and reassurance to all participants, volunteers, and staff; notifies the Division Director, in a timely way, of any problems or concerns; creates incident reports as necessary.
  • Gathers and provides data for outcomes and written reports requested by city, state, or federal funding sources.
  • May be required to cover Targeted Case Management responsibilities in order to maintain program operations and ensure the continuity of care of participants.
  • Other duties as assigned to fulfill MHP's mission.
