About The Position

This position provides leadership of two areas: Patient Safety and Process Improvement initiatives. Patient Safety: This person is responsible for working collaboratively with key leaders across the market and enterprise for the purpose of measurably improving patient safety and reducing patient harm/errors. This requires responsibility for performing Patient Safety Investigations and for supporting and performing patient safety event data analysis and activities that support UnityPoint Health (UPH) safety initiatives. This position assists in the resolution of safety issues/concerns by facilitating root cause analysis and action planning with cross-functional teams, leading them through process improvement activities utilizing evidence-based quality improvement tools and methodologies. Process Improvement: This person is responsible for helping to build an organization-wide culture of continuous improvement. To do this, this position partners with key stakeholders to lead improvements. Additionally. This position supports staff within each department as they embed improvement tools and methodologies into their culture. This position utilizes a variety of Six Sigma, Lean, Process Mapping and Change Management tools to accomplish these objectives. Requirements

Bachelor's degree in Nursing
Minimum of five years of healthcare experience.
Basic understanding of care processes provided in multiple sites of care.
Current licensure in good standing to practice as Registered Nurse in Iowa.
Valid driver's license when driving any vehicle for work-related reasons.
Excellent verbal and writing skills.
Advanced interpersonal communication skills to provide effective consultation and collaboration.
Ability to relate to clinicians and support staff with diverse backgrounds.
Ability to organize and define problems, develop strategies, and carry out action plans.
Analytical skills to interpret trends and analyze data.
Capable of exercising sound judgment and making independent decisions.
Ability to adapt to a complex organizational environment.
Use of usual and customary equipment used to perform essential functions of the position.

Nice To Haves

Master's Degree
Prior healthcare leadership responsibility. Knowledge of rapid cycle improvement theory and human factor principles.
Experience in patient safety or patient care performance improvement or relevant area.
Certified Profession in Patient Safety (CPPS)
Just Culture Certification
Six Sigma Black Belt, Lean or Adaptive Design certification

Responsibilities

Lead patient safety strategy / Resource for patient safety
Provides feedback to update the UPH system plan as needed.
Work closely with Risk Management, other market Patient Safety leaders, and UnityPoint Health Leadership, to conduct an annual assessment of patient risks and recommends patient safety goals.
Keep abreast of federal reporting regulations and guidelines related to patient safety.
Coordinate organizational response to regulatory standards related to patient safety including but not limited to DNV/CMS requirements.
Serves as Just Culture Champion and provides education as needed including new staff, leaders and others where needed.
Serves as a subject matter expert in patient safety science (ie Human Factors Analysis) and provides internal consultation and education/training as needed.
Facilitates prevention, identification and reporting of patient safety issues and problems, role-modeling nonpunitive approaches to improvement.
Clinical Safety Event Identification & Investigation
Reviews daily RL report listings for potential clinical Serious Safety Events (SSE) and initiates appropriate notifications and initial review.
Determines initial SSE event classification level.
Leads patient safety investigations including chart reviews, staff and provider interviews, collection/analysis of other relevant documents.
Identifies contributing factors such as staffing levels, environmental factors, human factors etc.
Root Cause Analysis Facilitation & Corrective Action Planning/Implementation
Leads and facilitates cross functional teams in the completion of RCAs for defined SSEs.
Assures completed RCAs including the Action Plan are shared with the Critical Event Committee (CEC) and when appropriate the UPH CEC.
Coordinates monitoring and action plan tracking for serious safety events and reports follow-up as appropriate.
Lead Process Improvement/Resource for Process Improvement
Partners with key stakeholders and managers and provides consultation as a subject matter expert.
Investigates core business processes and develops recommendations for improvement and facilitates implementation to resolve matters of significance on behalf of management.
Effectively facilitates a variety of audiences through a variety of methods.
Carries out major assignments in conducting operations by improving processes and efficiencies.
Partners with Analytics or Quality to support data collection and analysis.
Organizes work efforts and accountability in a project environment
Prepares documentation and supplies.
Process Improvement:
Training of Improvement Tools and Methodologies.
Strong understanding of process improvement tools, methodologies and UPH Management System.
Ability to mentor staff.
Easily translates process improvement to individual needs with staff.
