The Program Manager / RN for the S-O-R Program is a licensed nurse (RN or LPN) who serves as both the lead clinician and program manager for the S-O-R Care-Management initiative under the C-C-M-P (Care Management Network) grant funded by SAMHSA. This merged role combines nursing care responsibilities such as triage, care coordination, and patient education with program management functions, including supervision of staff, compliance oversight, and grant reporting. The Program Manager / RN ensures that clients, especially those experiencing homelessness and struggling with Substance Use Disorders (SUD) or other high-risk conditions, are connected to timely, high-quality medical, behavioral-health, housing, and supportive services.
