About The Position

The Program Manager Float will coordinate support for new people moving into a Life Unlimited Community Living Department. The caseload assigned to the Program Manager Float will fluctuate often based on the needs of the new individuals moving to the Community Living department and new Program Managers being hired within the agency. In addition to and/or in place of a caseload of individuals the Program Manager Float may also be assigned a caseload of Program Manager tasks or functions to complete. The Program Manager Float position will typically have a lighter caseload than other Program Managers to allow for the on boarding, development, and smooth transition of new individuals moving into LUs residential services and/or new LU team members providing services to a new ISL.

Requirements

  • Bachelors Degree in position related field or 5 years experience in the field of developmental disability services, preferably with supervisory experience.
  • Must obtain and maintain training and certification in all required areas such as Abuse/Neglect, CPR, First Aid, Level I Medication Administration, Positive Behavior Supports, TOOLS and Crisis Intervention.
  • Must have reliable personal vehicle for transporting clients.
  • Must obtain a Class E drivers license during new hire orientation must carry minimum coverage applicable by law for auto insurance
  • Must Receive Seasonal Flu Vaccination
  • Must possess good communication skills both orally and in written form and the ability to effectively and efficiently use computers or mobile devices to input, access, modify, or output information. This includes the ability to quickly access, store and retrieve data.
  • Must obtain and maintain training and certification in all required areas such as Abuse/Neglect, CPR, First Aid, Level I Medication Administration, Positive Behavior Supports, MANDT and MANDT Advanced Technical Skills.
  • Must have a desire to manage a team of staff supporting individuals with developmental disabilities and advocate for them in any possible way.
  • Must possess self-motivation, initiative and independence to work without direct supervision.
  • Must be flexible, reliable, and dependable.
  • Must be able to manage and embrace change, look for ways to improve own performance and accept feedback.
  • Must be willing to accept challenges of a 24-hour support system.
  • Ability to plan and prioritize to meet deadlines.

  • Background in the skills of developmental, implementation and the writing of teaching strategies preferred.

Responsibilities

  • Responsible for ensuring the Community Living program functions according to high quality standards.
  • Works with Intake Team on new residential individuals and locations preparing to open.
  • Develop a positive working relationship with new individuals, guardians, and SC to ensure a smooth transition into LU Services.
  • Works with the collective Intake Team to ensure home is within LU quality prior to individual(s) moving in.
  • Works with Intake Manager(s) on site development, roommate matching, and individual budget creation, to ensure a smooth transition to community living services.
  • Collaborate with the Recruiting and Training Team to ensure adequate staffing (as per Staffing Pattern) is available prior to the opening of the new residential location.
  • Work with and train new team members (Program Managers included) on the basic functions of the home, which includes but not limited to individuals outcomes, specific needs for the individuals served, Guardians goals for their individual, expectations for the home.
  • Ensure staff is trained and home is prepared for transition within 90 days of opening.
  • Conducts regular visits, at least 1x weekly, to homes on assigned caseload for Quality Assurance observation and structured feedback which includes but is not limited to financial audits, staff training, and face-to-face visits with individuals served until home is transitioned.
  • Train and support new Team Supervisor(s) ensuring that all agency and individual funds are recorded accurately and accounted weekly or as needed.
  • Must be familiar with completing an ISL budget, Staffing Pattern and reporting staffing variances to the Program Director.
  • Assist with any of the above items on caseloads assigned to other Program Managers when requested.
  • Will work a direct care shift at least six (6) times per year. Must work at least once per year in all homes on assigned caseloads. Ensures documentation is compliant will all applicable laws, rules and regulations.
  • Ensure confidential and health related records are kept secured.
  • Create individuals files and electronic records for new resident(s) to ensure all requirements are met.
  • Work with and assist Intake Team with scheduling medical appointments and ensure appointments are available in Therap.
  • Works with team(s) to ensure documentation of medical appointments, treatment plans and progress on health-related goals is completed according to policy and procedures.
  • Ensure RN recommendations via HRST/Therap are addressed within 30 days.
  • Support team(s) to ensure all appointments and all medical records are entered into correct locations in Therap. Responsible for Person Centered Planning process for assigned caseload.
  • Collaborate with the Intake Team and Support Coordinators to ensure 30-day Post Move planning meetings occur.
  • Participate in individuals 30-day Post Move meeting and ensure the following is available for the meeting: a collection and compilation of residential team feedback regarding individual transition before the meeting, contributing during the meeting, and suggesting ISP outcomes.
  • Advocate for individuals self-expression and ensures their hopes and dreams are addressed in their ISP or Addendum.
  • Collaborate with support coordinators to ensure feedback provided at Post Move meeting is accurately captured in ISP or Addendum. Follow up as necessary.
  • Provide support to program teams to ensure effective training in the implementation of individuals ISP, ISP Outcomes, and Behavioral Support Plan.
  • Identify new physicians (as needed) for individuals moving into services.
  • Responsible for supporting staff in following proper procedures for reporting of daily documentation into electronic client records management system.
  • Review shift notes weekly to ensure documentation is completed timely and individuals are making progress towards goals.
  • Complete Therap Monthly Summaries to report progress individuals are making towards their goals and outcomes. Maintains agency and individual funds assigned to work-site location.
  • Collaborate with Life Unlimited Finance Department to development of ISL budgets/staffing patterns, ensuring Master Schedule and Payroll System matches approved staffing patterns, and report to staffing pattern variances.
  • Ensure houses are staffed based upon the approved staff patterns and ratios.
  • Provides weekly oversight on all finance accounts/ledgers and ensures funds are accounted for according to agency policy.
  • Work with Team Supervisors to ensure receipts for purchases and individual paystubs are maintained as per agency policies and practices.
  • Work with Team Supervisor and finance department to ensure all NAFS funds are allocated appropriately. Responsible for ensuring the Community Living program is adequately staffed, trained and compensated.
  • Communicate any change in functioning, health status, or behavior to members of the program team, including on-call manager.
  • Responsible for facilitating monthly House and Staff Meetings for caseload.
  • Provide 24 hour on-call support to Team Supervisors on caseload
  • Support Team Supervisors in training of new hires at individuals homes on assigned caseloads.
  • Support Team Supervisors in providing performance feedback and corrective action.
  • Support Team Supervisors and the payroll department in the payroll approving process. Responsible for timely and ongoing communication and satisfaction.
  • Attending all Community Living meetings, in-services, and opportunities for on-going education as required.
  • Coordinate and conduct 60-day post move transition meeting with assigned Program Manager, Program Director and Intake Team (as needed) to ensure team is ready for 90-day transition.
  • Maintain professional relationships with guardians, family members, co-workers from other programs, and support coordination agencies frequently or according to their preferences.
  • Ensure individuals and guardians are satisfied with their services and are introduced to new Program Manager prior to 90-day transition.
