About The Position

The Program Director supervises a team of Care Coordinators and a Patient Navigator. As needed, the Program Director is also expected to perform the duties of Care Coordinators including, but not limited to coordination of services for an assigned caseload of enrollees. As this is an evolving program, additional responsibilities will be added.

Requirements

  • The position of Program Director requires a Bachelor’s degree in Social Work, Public Health, Health Administration, Human Services, or a related field
  • Minimum of 2 years of supervisory experience in care management, care coordination, or related healthcare or social service programs.
  • Must have exceptional communication skills both orally and in writing regarding complex and sensitive issues or regulations.
  • Excellent coaching, supervision, and team development skills.
  • Must have very good computer skills (word and excel).
  • Physical ability to perform the essential job functions are needed.

Nice To Haves

  • Master’s degree preferred.

Responsibilities

  • Provide case specific supervision to the care management team to ensure coordination of concrete services with HIV, medical, mental health, housing, and social service providers.
  • Handle case conferencing with providers to determine goals/objectives, activities and who will perform needed tasks related to mental health issues for member and family.
  • Assist staff to increase member’s health literacy around the purpose of mental health and substance use treatment, also the use of psychotropic medications.
  • Assist staff to identify and provide referrals to mental health and substance abuse services.
  • Identify barriers that impede a member’s engagement in treatment and assist team to develop service plans to reduce/eliminate barriers
  • Observes home visits and face to face meetings and is directly involved in coordination of care to higher risk cases.
  • Review members for continued eligibility for Health Homes; assist staff to develop a graduation plan for all members and monitor member milestone achievements.
  • Ensure comprehensive and supportive on-boarding of new hires.
  • Provide ongoing training to care management staff to enhance team performance.
  • Oversee the quality, accuracy, and timeliness of documentation, including assessments, care plans, progress notes, BSQ’s and service verification forms.
  • Monitor program dashboards and performance metrics by utilizing tracking tools and completing monthly reports, dashboards and attestation forms.
  • Conduct regular individual supervision, team meetings, and performance reviews; address performance issues through coaching and corrective action as needed.
  • Provide oversight for Transitions of Care Process and tracking, implementing strategies to prevent readmissions and reduce hospitalizations and unnecessary ER visits.
  • Ensure that care management activities and operations are in compliance with federal, state, and local health care regulations and standards of Coordinated Behavioral Health (CBC).
  • Ensure audit readiness and support internal and external audits, chart reviews, and service verification audits on a monthly basis.
  • Develop and support relationships with providers that result in continued improvement in quality healthcare outcomes.
  • Drives efforts to engage providers in collaborative efforts during care transitions.
  • The Program Director shall perform any duties as required to meet the needs of the program and the clients we serve.
