About The Position

The Program Director is the leader of the Targeted Case Manager team (TCM from this point forward). TCM provides medically necessary services to individuals over the age of 18, has a diagnosis of a serious mental illness, and assists with access to medical, social and other services, such as acquisition of resources and ongoing support for retention and engagement of outpatient mental health treatment, while serving as a coordinator for outpatient care with a person-centered lens. The Program Director is responsible to review initial intakes, assign cases to their targeted case management team, oversee with consistent supervision with staff and partnership with the QA committee that all areas of R79 case management is followed. The Program Director will also carry a small case load. The Program Director will be expected to oversee the day-to-day tasks/functioning that the case managers are engaged in as well as offer ongoing consultation, direction/feedback and problem solving. The Program Director is expected to be knowledgeable about processes that exist in Hennepin County and will stay current with all trainings and meetings that supports this endeavor. Requirements

  • It is highly recommended that the Program Director hold a master’s and holds a clinical license in one of the following fields: social work, clinical counseling, or marriage and family therapy from an accredited college or university.
  • Pre-licensed/trainee status individuals may be considered on a case-by-case basis.
  • Experience of 2,000 hours or greater in community mental health will be highly considered.

Responsibilities

  • Oversee the day to day functioning of the TCM team on a day-to-day basis (and intentionally scheduled, supervised time)
  • Support with conducting a variety of clinical assessments to determine needed areas of the ICSP (Individual Community Support Plan)
  • Monitoring intakes and assigning them to TCM’s on the team
  • Monitoring discharges/transfers when TCM is no longer the recommended service for the person served
  • Knowledgeable about Civil Commitment responsibilities
  • Building meaningful relationship; knowledge and skill with benefit acquisition and management; resource and referral; knowledge of Rule 79 clinical documentation.
