The Project Coordinator for the Hepatitis C Grant is responsible for day-to-day management and operational coordination of the Hepatitis C Elimination Initiative Pilot at Hillcrest Family Services. This position plays a key leadership role in implementing integrated behavioral health and infectious disease services - focusing on same-day HCV/HIV testing, care coordination, and linkage to treatment for individuals with or at risk of Hepatitis C, Substance Use Disorder (SUD), Serious Mental Illness (SMI), and homelessness. Working closely with the Project Director and Nurse Care Manager, the Project Coordinator ensures effective service delivery, staff supervision, compliance with SAMHSA requirements, data reporting, and partnership engagement to achieve the program's goal of eliminating HCV among high-risk populations.
