About The Position

Keystone Human Services is currently seeking Program Coordinators to join our team in providing individualized, person-directed support in the area of mental health and intellectual disabilities. The Program Coordinator supports individuals with disabilities by teaching, coaching, and modeling the skills needed to participate fully in family, community, cultural, and civic activities. The role focuses on helping individuals build meaningful relationships, make informed choices, and engage in experiences that promote independence and personal growth. Requirements

  • One of the following: High school diploma or equivalent with three months of related work experience; Associate or bachelor's degree with related work experience
  • Twenty-one years of age or older
  • Program administrator certification or ability to obtain certification within six months
  • Effective communication and proficient computer skills
  • Valid driver’s license with daily access to a privately maintained and insured vehicle
  • Successful completion of the pre-employment process, including clearances and verifications

Responsibilities

  • Support individuals in accessing community resources, building life skills, and increasing participation in meaningful daily activities
  • Provide training, guidance, and direct support to enhance personal competencies and promote social integration
  • Deliver direct care support based on program needs
  • Provide transportation for individuals as needed
  • Perform general housekeeping tasks and maintain cleanliness of the home and property
  • Maintain inventory of household equipment and furnishings in accordance with agency standards
  • Mentor and train new employees, including orientation support and documentation coaching
  • Assist with schedule development, maintenance, and coordination of daily activities
  • Review, validate, and approve service documentation and daily notes to ensure accuracy and compliance with individual outcomes
  • Support supervisors with timely and accurate billing processes
  • Coordinate and attended medical appointments, ensuring follow‑through on physician orders
  • Complete all required documentation within established timeframes.
  • Follow all medication administration and monitoring procedures, including required documentation
  • Maintain organized record‑keeping systems, including Person-Centered Planning, treatment documentation, and discharge planning
  • Collaborate with families, support professionals, and interdisciplinary team members to ensure cohesive service delivery
  • Coordinate assessments, support plans, and implementation of individualized services
  • Participate in on‑call rotation as required
