The Enhanced Care Management (ECM) program provides person-centered care coordination to individuals with complex medical, behavioral health, and social needs. Services are delivered by a multidisciplinary team and include care coordination, referrals, advocacy, and linkage to medical, behavioral health, and community resources. The program emphasizes whole-person, culturally responsive, and trauma-informed care to improve health outcomes and member stability.

Requirements

  • Bachelor’s degree in psychology, child development, behavioral science, social work, criminal justice or related field, preferred.
  • Minimum of 2 – 3 years of experience providing services and care coordination, preferably Enhanced Care Management (ECM), to individuals with complex medical, behavioral health and social needs.
  • Experience should preferably have been in a community based or neighborhood service organization.
  • A minimum of two years of experience in a supervisory role, preferred.
  • Must possess initiative, ability to set goals, and prioritize while being highly organized.
  • Must possess strong critical thinking, problem solving skills and handle crises effectively.
  • Must possess strong oral and written communication skills and ability to speak publicly.
  • Must possess good working knowledge of Microsoft Windows, including Outlook, Word, and Excel to develop, update, maintain and manage spreadsheets and data reports.
  • Ability to work with a diverse population, possess cultural sensitivity, and maintain appropriate boundaries with clients.
  • Employment may be conditioned upon satisfactory completion of certain background clearances which may include a Department of Justice criminal background check requiring my fingerprints (Live Scan), screening through the National Sex Offender Public Website, drug test, tuberculosis (TB) test, and such additional clearances as may be required under the program.
  • Candidate is required to have reliable transportation; therefore, the candidate must successfully complete a motor history check.
  • Candidate must also possess and maintain a current valid California driver’s license and valid auto insurance policy.

Responsibilities

  • Manage the daily operations and overall administration of the Enhanced Care Management (ECM) team and program.
  • Provide day-to-day coordination and oversight of ECM services to ensure compliance with contractual, County, and agency requirements.
  • Supervise and support assigned program staff, including onboarding, training, ongoing coaching, performance management, and evaluations.
  • Responsible for facilitating the development and implementation of team goals and objectives.
  • Foster a positive, collaborative team environment and ensure high-quality service delivery through consistent supervision and leadership.
  • Monitor and track staff productivity, caseloads, and billable units; regularly review productivity reports and address trends, gaps, or compliance concerns in a timely manner.
  • Serve as the primary lead for program data entry, data validation, and database management to ensure accuracy, completeness, and timeliness across systems.
  • Maintain oversight of electronic health records and program databases, conducting routine quality assurance reviews and resolving documentation errors or discrepancies.
  • Prepare and manage accurate and timely invoicing, including reconciling service data and validating billable units.
  • Collaborate closely with Program Directors to promote program awareness and support ongoing referrals.
  • Support staff in understanding ECM requirements, documentation standards, workflows, and billing-related expectations.
  • Participate in meetings and planning efforts to support ECM program growth, including expansion of Community Health Worker, Justice-Involved services, Community Supports, and Housing Assistance.
  • Oversee and ensure completion of monthly statistical service reports as required by SBCS.
  • Maintain strict adherence to agency and County confidentiality and privacy standards for protected health information (PHI) in accordance with HIPAA and applicable laws and policies.
  • Attend and participate in Leadership Team meetings, team meetings, department meetings, agency meetings, and committees as assigned.
