About The Position

The Program Coordinator will manage care coordinators, ensuring all documentation is reviewed and deadlines are met. This role involves completing monthly reports, conducting regular quality assurance checks, and leading weekly team meetings. The coordinator will supervise 1-2 teams, review documentation for timeliness and quality, maintain caseload lists, perform data analysis, assign new cases, and ensure compliance with program standards. They will also act as a consultant for team members, address client concerns, conduct weekly supervision, attend administrative meetings and training, participate in program development activities, and handle payroll, billing, and performance evaluations.

Requirements

  • Master's degree in the social services field plus two years' experience in human services or bachelor's degree in social services field plus 3 years' experience in human services.
  • At least 1 year supervisory experience
  • Valid and clean driver's license

Responsibilities

  • Supervise 1-2 teams, consisting of a Senior Care Coordinator and up to 10 Care Coordinators
  • Review submission of all documentation in client charts (including progress notes, intake assessments, annual updates and discharge documents) for all team members, including timeliness of submission as per health home deadlines as well as for content and quality of services provided
  • Complete monthly reports pertaining to service provision and client status for the entire team to be submitted to the Division Director
  • Maintain caseload lists for all team members, including tracking of enrollments, closures, updating of care plans, and changes in managed care status/plan and Medicaid recertification.
  • Ensure all changes are reported in a timely manner and submit updated caseloads to the Division Director weekly
  • Perform regular data analysis of various documentation reports obtained through the EMR pertaining to performance and submission
  • Assign all new cases to appropriate Care Coordinators
  • Conduct regular quality assurance checks on client records to ensure that all information is entered and all records are kept to program, agency, health home and county standards.
  • Conduct weekly team meetings to review and to relay information regarding program changes/updates to policy, share new resource information with team
  • Act as a consultant for all members of the team to provide information regarding community resources, social welfare programs and other information, as needed
  • Address client concerns and/or grievances that are not able to be resolved directly.
  • Conduct weekly supervision with each Care Coordinator to address needs and concerns/behavior issues and professional skills development
  • Attend administrative meetings and supervision with the Division Director to discuss program issues/concerns, policy changes, relay any staff concerns and work on supervisory skills development
  • Attend training to enhance skill levels and remain abreast of system changes (including MAPP, HARP/CORE)
  • Participate in activities through the health home and other sources to enhance program development and to assist with developing improving systems of care coordination service delivery
  • Complete weekly payroll, monthly billing, probationary and annual performance evaluations for team members
  • Perform any additional tasks assigned by the Division Director
