About The Position

Under the guidance of the Program Manager, the Program Coordinator will play a key role in identifying, educating, and navigating individuals in need of cancer screening and follow-up services. This position involves working closely with our Cancer Patient Navigation team, Primary Care Providers, and community-based organizations to maximize impact and reduce health disparities. You will be responsible for coordinating and implementing education, outreach, and administrative tasks, including identifying patients in need of screenings, assisting with appointments, and addressing barriers to care. The role also involves managing data, reporting outcomes, and collaborating with case managers and clinical partners to ensure program efficiency, improve patient adherence, and enhance overall community health. Your efforts will contribute to providing innovative care and support, extending critical health services to hundreds of individuals while reducing disparities in cancer care. This is a grant funded position.

Requirements

  • Bachelor’s degree or 3-5 years of equivalent work experience
  • Proficiency with Microsoft Office
