About The Position

University of Washington Medical Center-Montlake's Kidney Transplant has an outstanding opportunity for a Program Coordinator-Patient Scheduler. This is a 100% FTE, Monday - Friday day shift position within Kidney Care & Transplantation Services, which also includes the General Nephrology Clinic at UW Medical Center – Montlake. The department treats patients with all forms of kidney disease, offering state-of-the-art care, access to new treatment studies, and traditional therapies. UW Medicine’s transplant team is experienced at planning and performing sophisticated living-donor transplants, with nephrologists and surgeons having over 10 years’ experience in living donation. The clinic also provides specialized care for genetically transmitted kidney diseases, high blood pressure, kidney stones, and kidney disease in the context of pregnancy, diabetes, and cancer. Requirements

  • High school graduation or equivalent
  • Two years of experience in the program specialty
  • Equivalent education and/or experience may substitute for minimum qualifications except when there are legal requirements, such as a license, certification, and/or registration

Responsibilities

  • Develop and maintain a systematic process to identify, analyze and implement standards to ensure patient continuity and service during post transplant period
  • Assess processes, communicate observations, concerns and ideas to the Transplant Team in a timely and professional manner to maintain high standards of patient care
  • Ensure that required information on each patient is entered into program database to update and monitor clinical statuses of post transplant patients
  • Monitor kidney organ acquisition budget, submit receipts, deposit payments and interface with and collect data
  • Coordinate extensive diagnostic tests necessary for evaluation and facilitate continuum of care for post kidney transplantation patients
  • Responsible for continuum of care steps preceding and following initial post-transplant appointment: test, procedure and appointment scheduling; outgoing referrals and coordination of care with PCP/outside nephrologists’ offices or other specialty care, and completion of missing outside provider documentation
  • Coordinate the receipt of requirements: patient forms, medical records, lab and diagnostic results from UWMC, primary care physicians’ offices, nephrologists’ offices and other medical facilities
