About The Position

This position serves as the Evaluator within the Community Clinical Connections for Prevention and Health (CCCPH) Branch and is responsible for leading program evaluation efforts for CDC-funded public health initiatives. The role includes planning, implementing, and managing evaluation systems that track performance measures related to obesity, nutrition, physical activity, diabetes, heart disease, and stroke prevention. The Evaluator develops data collection tools, analyzes both qualitative and quantitative data, and presents findings to internal staff and external partners. This position ensures alignment between performance measures and program workplans and supports continuous improvement through technical assistance and training. The Evaluator works closely with program staff, contractors, and stakeholders to ensure timely and accurate reporting of evaluation deliverables.

Requirements

  • Bachelor's degree from an appropriately accredited institution and three (3) years of experience in the program related to the area of assignment

  • An equivalent combination of education and experience.

  • Demonstrated experience designing and managing evaluation systems using tools such as Access, Excel, FormStack, Survey Monkey, and Power BI to collect, track, and report program performance data.
  • Proficient in collecting, cleaning, and analyzing both qualitative and quantitative data to support annual reporting requirements for federally funded public health programs.
  • Skilled in developing logic models, evaluation plans, and performance measures aligned with program workplans focused on chronic disease prevention, including obesity, diabetes, and heart disease.
  • Experienced in presenting evaluation findings and trends to internal and external stakeholders using data visualization tools, reports, maps, and PowerPoint presentations.
  • Proven ability to provide technical assistance and training to program staff and partners on evaluation methods, data interpretation, and continuous quality improvement strategies.

Responsibilities

  • Lead the development and implementation of evaluation plans, logic models, and performance tracking systems for CDC-funded public health programs.
  • Design and manage data collection tools and systems using platforms such as Access, Excel, FormStack, Survey Monkey, and Power BI.
  • Collect, clean, and analyze qualitative and quantitative data to support annual reporting of performance measures.
  • Align evaluation metrics with program workplans by collaborating with program staff and partners to identify appropriate indicators and data sources.
  • Prepare and submit evaluation deliverables including annual evaluation plans, reports, and performance measure updates for CDC cooperative agreements.
  • Present evaluation findings, trends, and recommendations through reports, presentations, maps, and data visualizations.
  • Provide technical assistance and training to program staff and partners on evaluation methods, data interpretation, and reporting tools.
  • Maintain and improve monitoring systems that summarize incoming data and support program decision-making.
  • Support contractors and funded counties in compiling and submitting required data and generating progress reports.
  • Oversee all CCCPH Branch evaluation and surveillance activities to ensure consistency, quality, and compliance with federal requirements.
