Program Coordinator – Health Education & Outreach – Family Planning Practice & Young Men’s Clinic The Health Education & Outreach Program Coordinator will oversee education and outreach activities for the Family Planning Practice and Young Men’s Clinic (FPP/YMC). The Coordinator leads the Health Education team at FPP/YMC and collaborates with key staff in other disciplines (medical, social work, benefits assistance, administrative) to deliver effective health education programming for FPP/YMC patients and the broader community. The Coordinator works with the Program Manager for Community Health Education to develop, implement, and evaluate the Community Education, Participation & Engagement plan, and nurtures a strong network of relationships with community-based organizations, schools, hospital programs, and city offices to provide culturally competent education to men, women, and adolescents in the community and link them to care at FPP/YMC. This position will require occasional travel to various sites across the city, mostly in northern Manhattan. Occasional evenings and Saturdays may be required to support outreach activities and events.

Requirements

  • Bachelor’s degree or 3-5 years of equivalent work experience
  • Proficiency with Microsoft Office
  • Strong interpersonal skills, highly organized, ability to work with diverse community groups and multi-task
  • Excellent writing and communication skills – possess verbal and written skills to convey mission and purpose

Nice To Haves

  • Strong background in reproductive and sexual health
  • Experience working with adolescents
  • Project management or project coordination experience
  • Bilingual English/Spanish

Responsibilities

  • Coordinating and documenting all community engagement activities, including presentations, workshops, open houses, clinic tours, and participation in health fairs and events
  • Coordinating and documenting inreach activities, including focus groups, surveys, patient advisory groups, and educational activities
  • Building and leveraging strong, mutually beneficial relationships with community partners (ex: NMIC, The Brotherhood Sister Sol, El Nido de Esperanza, Life Is Precious, Harlem Community Justice Center, Getting Out Staying Out, Fresh Youth Initiatives, CUNY Fatherhood Program) and other NYP programs (ex: School-Based Health Centers, NYPeers, Uptown Hub, Lang Youth Medical Program, HEAL)
  • Facilitating presentations and workshops for a variety of audiences, including program staff, youth, and adults
  • Serving as the adolescent patient liaison and assisting with connecting young patients to care, including manning an online contact form and working with administrative staff to fast-lane patients for appointments when needed
  • Assisting Program Manager to ensure compliance with Title X and New York State Family Planning Program requirements, including leading annual trainings and tracking staff member completion of individual requirements
  • Supporting Program Manager with quarterly and annual program reports
