About The Position

THE PROGRAM COORDINATOR IS RESPONSIBLE FOR THE DAILY OPERATION OF THE ASSIGNED VOCATIONAL PROGRAM, DAY PROGRAM, AND COMMUNITY PARTICIPATION PROGRAM. SHE/HE WORKS TO ENSURE THAT SERVICES ARE DELIVERED IN ACCORDANCE WITH AN INDIVIDUAL’S PLAN. THE PROGRAM COORDINATOR IS ALSO RESPONSIBLE FOR ENSURING THAT AGENCY AND REGULATORY GUIDELINES ARE MET.

Requirements

  • Bachelor’s degree. Degree in a social services discipline preferred.
  • Minimum of 5 years’ experience working in the field of developmental disabilities with at least 3 years of supervisory experience.
  • Strong written and verbal communication skills
  • Ability to read and write in English

  • proficient interpersonal relations and communicative skills
  • auditory and visual skills
  • Valid Driver’s license with a clear three-year driving record
  • Ability to perform the physical duties required of Direct Care Worker.
  • Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in providing various care; and operating a vehicle.
  • Must be able to regularly lift and/or move up to 40 pounds and occasionally must lift and/or move up to 50 pounds, including transferring of client.
  • Satisfactory completion of post-offer pre-employment physical exam and drug test as required. Completion and clearance of a criminal background check
  • Must maintain skills necessary to safely and efficiently operate the following equipment and machines used in the performance of this job:

Responsibilities

  • Provides leadership and management to staff who support individuals in their vocational, day and community support programs.
  • Oversees the implementation of programming and services for all individuals, including life skills, habilitative activities, vocational training, and job placement.
  • Provides guidance and supervision to the Program Specialists Supervisor and the Program Supervisor.
  • Monitors service delivery and staff interactions through direct observation and monitoring of assigned programs and review of documentation.
  • Works collaboratively with Program Specialists to develop individuals’ supports plan.
  • Manage the program vehicle fleet.
  • Advocates for and encourage the growth and development of individuals served.
  • Oversees the hiring, training, supervising and the evaluation of the performance of staff.
  • Oversees the marketing of services to potential consumers and community partners and civic organizations.
  • Explores opportunities for job placement, volunteerism, and community involvement for individuals.
  • Maintains and increases revenue through the efficient use of resources.
  • Participates in the assessment and admission of individuals.
  • Regularly review individual progress to ensure that they are appropriately placed.
  • Regularly review program expenses and petty cash fund.
  • Is knowledgeable of, and complies with, all relevant agency policies as well as State and Federal Regulations; including but not limited to; Wage and Hour Regulations; PA Code 2380 and 2390 Regulations and regulations related to compliance with the CMS Waivers
  • Actively participates in all aspects of the licensing and recertification process.
  • Actively participate in the Incident Management process, to include the management and reporting of incidents as occurring; maintaining Investigator certification; and participating in Point Person/Investigator on-call as assigned.
  • Serves as the Program Specialist for assigned individuals when necessary.
  • Assists the Director or Director’s Designee with various administrative tasks to support the operation of the division.
  • Serves as an agency liaison between county/state agencies.
  • Attends county, regional and state meetings as assigned.
  • Meets and maintains required training and certifications.
  • In the absence of the Director or Director Designee, the Coordinator will assume program duties as assigned.

  • Is knowledgeable of Individual rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all individuals served in a safe, secure environment.
  • Supports, cooperates with, and implements specific procedures and programs for:
  • Safety, including staffing ratio, staffing assignments, universal precautions and safe work practices, medication administration, established fire/safety plans, report and/or correct unsafe working conditions, equipment repair and maintenance needs.
  • Confidentiality of all data, including individuals served, employee and operations data.
  • Compliance with all regulatory requirements and agency policies as required by Human Resources, licensing, and funding agencies.
  • Demonstrate knowledge and compliance with HIPAA standards.
  • Conducts oneself in a professional manner when representing Indian Creek Foundation.
  • Functions well as a member of the Day Management Team:
  • Cooperate and work together with all co-workers; plan and complete job duties with minimal direct supervision, including appropriate judgment.
  • Use tactful and appropriate communications in all situations.
  • Report and follow up regarding reported complaints, problems, and concerns with appropriate personnel.
  • Promotes positive public relations with individuals served, family members, other team members and community.
  • Completes requirements for training, acceptable attendance, dress codes including personal hygiene, and other work duties as assigned. Follows the Code of Conduct in the performance of duties.
  • Complete all documentation to ensure compliance with all regulations and agency policies.
  • Completes annual compliance HIPAA training and exhibits behavior as set forth by the Code of Compliance in performance of duties
