Program Assistant (Per-Diem) - Adult Day Health

About The Position

Vitra Health is hiring, and we believe that our team is the key to our success! We are looking for a Program Assistant to join our team, in our Adult Day Health Center in Brockton. The Program Assistant will support Vitra’s Adult Day Health (ADH) center in Brockton, MA. This is a daily care facility designed to meet the physical, functional, and social needs of seniors or individuals with disabilities, including those with complex medical or rehabilitative needs.

Requirements

Active Massachusetts' CNA certification or HHA certification
Previous experience in nursing home strongly preferred.
Ability to read and follow written instructions and documents.
Has empathy for the needs of the clients.
CPR certification or willing to obtain.
One or more years of experience working with adults in a healthcare or social service setting.

Responsibilities

Assist Adult Day Health care members in personal care activities such as hygiene, grooming, dressing and personal care as needed.
Assist members in the practice of safety and good nutrition when is possible.
Adhere to the documentation and care procedures and standards of personal and professional conduct.
Observe the patients for changes in general physical and mental conditions.
Report any changes noted to the Adult Day RN.
Assist and supervise activities of daily living.
Assist with all the activities programs including arts, crafts and other small group activities under the direction of the Activity Director and/or Program Director.
Assist with trips and other community outings outside the program.
Participate in staff meetings, in-service training and other relevant training.
Prepare, serve and clean up before and after meals.
Other duties as assigned
