Program Assistant (Per-Diem) - Adult Day Health
About The Position
Vitra Health is hiring , and we believe that our team is the key to our success! We are looking for a Program Assistant to join our team, in our Adult Day Health Center in Brockton. The Program Assistant will support Vitra’s Adult Day Health (ADH) center in Brockton, MA. This is a daily care facility designed to meet the physical, functional, and social needs of seniors or individuals with disabilities, including those with complex medical or rehabilitative needs. Requirements
- Active Massachusetts' CNA certification or HHA certification
- Previous experience in nursing home strongly preferred.
- Ability to read and follow written instructions and documents.
- Has empathy for the needs of the clients.
- CPR certification or willing to obtain.
- One or more years of experience working with adults in a healthcare or social service setting.
Responsibilities
- Assist Adult Day Health care members in personal care activities such as hygiene, grooming, dressing and personal care as needed.
- Assist members in the practice of safety and good nutrition when is possible.
- Adhere to the documentation and care procedures and standards of personal and professional conduct.
- Observe the patients for changes in general physical and mental conditions. Report any changes noted to the Adult Day RN.
- Assist and supervise activities of daily living.
- Assist with all the activities programs including arts, crafts and other small group activities under the direction of the Activity Director and/or Program Director.
- Assist with trips and other community outings outside the program.
- Participate in staff meetings, in-service training and other relevant training.
- Prepare, serve and clean up before and after meals.
- Other duties as assigned
