About The Position

Assists the Activity Coordinator in the planning, developing, organizing, implementing, evaluating, and directing the activity/engagement events for the long term care facility. Transports and/or arranges transportation services for the residents safely and efficiently. Provides a clean and caring atmosphere for the residents and assists with activities of daily living and recreational activities.

Requirements

  • Two years of work experience in a healthcare setting required.
  • Certified Nursing Assistant in Commonwealth of Kentucky required.
  • Valid Kentucky driver's license required.
  • Motor vehicle record check required and will be performed annually.

Nice To Haves

  • Long Term Care experience preferred.
  • Associate's or Bachelor's degree preferred.
  • Certification through the National Certification of Activity Professionals preferred.

Responsibilities

  • Performs resident care duties (i.e. serving and recording meals, toileting, and assistance with ambulation) with attention to resident safety and comfort.
  • Operates the facility vehicle in the delivery of residents with attention to safety and comfort. Operates wheelchair lift. Assists residents in and out of vehicle utilizing proper body mechanics.
  • Adheres to driving laws and regulations and assures safe operation of vehicle. Conducts general maintenance checks on company vehicle. Reports maintenance issues. Maintains record of maintenance checks.
  • Visit residents and carries out individualized Plan of Care for Activities and Engagement.
  • Facilitates daily engagement activities.
  • Maintains cleanliness and working condition of facility vehicle in accordance with owner's manual and with regard to safety standards.
  • Communicates with cognitively impaired, aphasic or other residents with impaired communication according to the plan of care.
  • Assist in the scheduling and planning of the monthly activity calendar. Post monthly calendar in each resident's room.
  • Assist with the direction and supervision of volunteers assisting with activities.
  • Assist the Activity Coordinator in maintaining records related to activities, assessments, progress notes, and resident activity participation records.
  • Assists with the completion of required documentation for Resident Council Meetings.
