About The Position

As a Program and Data Analyst I, you will contribute to the mission, vision, and values at Primary Health Care, Inc. by supporting health center staff in continuous improvement through the use of relevant data reports. Primary responsibilities include assisting with the management, analysis, and interpretation of data to support quality initiatives and metrics that align with the Performance Improvement Plan at PHC. In this role, the analyst gains experience working with key data systems and collaborates with clinical and non‑clinical teams to ensure accurate, timely reporting. Demonstrates PHC’s iCare values in daily work.

Requirements

  • Bachelor’s Degree in healthcare, clinical analytics, informatics, data management or other related field. Equivalent experience will be considered in lieu of the required education.
  • A minimum of 1 years’ experience working with healthcare data, clinical quality, and performance improvement outcome measurement.
  • A minimum of 1 years’ minimum experience in data management, reporting, and analysis tools and techniques
  • Experience organizing and presenting information and data to diverse audiences.
  • Experience with and understanding of data concepts including data management and stewardship.
  • Excellent computer skills with proficiency in Microsoft Office programs including, Word and Excel along with other software or data platforms.
  • Excellent communication skills, verbal and written, with ability to effectively communicate technical information to non-technical people.
  • Organization, prioritization and time management skills with detail orientation and ability to multi-task.
  • Strong interpersonal skills.
  • Strong analytical and problem-solving skills with ability to think critically and creatively using vision independent judgement.
  • Flexible with ability to adapt to ongoing work/project changes.
  • Team orientation with ability to work collaboratively with others.

Nice To Haves

  • Experience working in primary care, specifically a Federally Qualified Health Center, Community Health Center, or Rural Health Center.
  • Fluency in spoken English and Spanish, with the ability to communicate clearly and accurately in both languages.

Responsibilities

  • Aggregates, analyzes, and reports health center data via summary reports, dashboards, and other reporting processes. Establishes and maintains reporting and communication tools, dashboards and data displays appropriately for customer needs including, but not limited to clinic support staff, clinical staff, management staff, committee members, executive staff, and/or the Board of Directors.
  • Utilizes data analytics to make recommendations on and support performance improvement projects for PHC.
  • Assists with Population Health Management Systems, Data Warehouse applications, Electronic Health Record reports, and other reporting data systems.
  • Conducts ongoing data stewardship and data validation. Maintains applications, including data entry, to ensure current and accurate data is available for specified patient groups. Audits documentation and system quality assurance.
  • Maintains knowledge of various regulatory agencies that drive data needs for PHC.
  • Assists clinics and programs on documentation workflow, data entry, and data extraction needs related to the use of key HIT systems; systems may include but are not limited to a population health system, data warehouse, electronic health records, government and grant reporting systems. Monitors and provides data updates to the staff members accountable to grant work plans. Assists staff with workflow development in accordance with quality initiatives.
  • Collaborates with leadership to develop quality assurance tools used to measure staff performance and documentation oversight. Educates and assists team members on proper electronic medical record documentation and recommends data capture improvements. Demonstrates knowledge of Lean principles to conduct PDSA cycles on applicable projects.
