About The Position

Home Care Network is seeking a highly skilled and experienced Program Administrator / RN Case Manager to join our home health agency and direct the business, care team, and agency growth of our Southwest Ohio operations, serving Dayton, Cincinnati, Columbus, and surrounding areas. DETAILS JOB TITLE: Program Administrator / RN Case Manager (RN required) JOB LOCATION: Dayton, OH; Cincinnati, OH; Columbus, OH; and surrounding areas (up to 2hr travel radius) Typical RN Case Manager in-field activities and services and state survey requirement require travel within a 30-60 minute radius, up to 2 hours occasionally / as needed. This role will have access to our Corporate Office in Dayton, OH, located at 10552 Success Lane, Ste M, Dayton, Ohio 45458. SCHEDULE: Monday - Friday from 8 AM - 5 PM and as needed/on call Field work + client home visits Monday-Friday as needed (with the ability to work from home 1-2 days per week when not required in the field). Willingness to provide occasional on-call and varied / flex hour coverage (late night / weekend emergency) is required as needed. Ability to report to our Jackson, OH Office in person as needed during State Surveys is required. RNs, LPNs, Home Health Aides, and Personal Care Assistants) while directing, coordinating, supervising, and implementing professional and supportive services to home health clients in a safe, cost-effective manner. Successful candidates will be professional, efficient problem solvers who work independently as well as on a team, have a positive attitude, attention to detail, and a bias for action and driving growth. Details: This is a full-time, salaried position requiring a blend of clinical, compliance, and business growth skills. Minimum availability requirements: This role will oversee clients and field staff across Southwest Ohio operations, serving Dayton, Cincinnati, Columbus, and surrounding areas. Requirements

  • Graduate of an accredited nursing program with a current license as a Registered Nurse in the state(s) of practice.
  • Minimum of 2 years Nursing experience required, with a combination of acute care and home health care/public health preferred.
  • 1+ year of Supervisory / DON / Team Management experience required (of caregivers, nurses, and care team), with health service administration a plus.
  • CPR & First Aide Certification required
  • Experience managing department / regional budgets, financial reports, and growth plans.
  • Extensive familiarity with HIPAA regulations and medical terminology.
  • Excellent written and verbal communication skills and strong interpersonal skills.

Nice To Haves

  • Baccalaureate degree in nursing or related field preferred.
  • CPR & FA Instructor/Trainer certification preferred (or willingness to complete once upon hire).

Responsibilities

  • Team Leadership & Staff Management Supervise and support a multidisciplinary team of RNs, LPNs, Home Health Aides, and Personal Care Assistants.
  • Recruit, hire, onboard, and retain field staff to meet client care needs.
  • Deliver effective orientation, ongoing education, and in-service training to ensure staff competency and regulatory compliance.
  • Conduct skills assessments and performance evaluations for clinical staff.
  • Foster a cohesive, collaborative care team environment.
  • Clinical Oversight & Client Care Perform client assessments and develop individualized care plans.
  • Promote continuity of safe, appropriate care through effective case management.
  • Coordinate and supervise the delivery of professional and supportive services to clients across all age groups (infant to geriatric).
  • Ensure timely communication of patient status and changes in condition to the care team.
  • Occasionally provide direct care, emergency support, or coverage in the field as needed.
  • Compliance & Quality Assurance Ensure compliance with state licensure, Medicare certification, and other regulatory requirements.
  • Develop, implement, and monitor policies and procedures to maintain high standards of care.
  • Lead quality improvement initiatives and recommend policy updates to enhance service delivery.
  • Strategic Planning & Program Development Participate in corporate and regional strategic planning to support agency growth and service excellence.
  • Design and execute initiatives aligned with diagnosis-specific care needs and business goals.
  • Manage specialty payor programs in your region, ensuring quality and operational efficiency.
  • Financial & Operational Management Monitor and manage departmental budgets and monthly financials to meet strategic objectives.
  • Assist in the development of annual operating and capital budgets.
  • Oversee staffing and program coordination to optimize resource utilization and client satisfaction.
  • Communication & Collaboration Facilitate effective communication among patients, families, physicians, and care teams.
  • Serve as a liaison to educate clients and families on available services and community resources.
  • Promote a culture of compassion, safety, and excellence in care delivery.
  • Acting in accordance with the Nurse Practice Act, as well as basic principles and best practices of professional nurses, a successful Administrator / RN Supervisor ensures that patients receive the best medical care possible by overseeing their well-trained team of nurses and aides.
  • Their purpose is to maintain safety and patient confidence by ensuring all patients' needs are addressed with the highest degree of professionalism and care, and that all policies and regulations are adhered to strictly.
