Home Care Network is seeking a highly skilled and experienced Program Administrator / RN Case Manager to join our home health agency and direct the business, care team, and agency growth of our Southwest Ohio operations, serving Dayton, Cincinnati, Columbus, and surrounding areas. DETAILS JOB TITLE: Program Administrator / RN Case Manager (RN required) JOB LOCATION: Dayton, OH; Cincinnati, OH; Columbus, OH; and surrounding areas (up to 2hr travel radius) Typical RN Case Manager in-field activities and services and state survey requirement require travel within a 30-60 minute radius, up to 2 hours occasionally / as needed. This role will have access to our Corporate Office in Dayton, OH, located at 10552 Success Lane, Ste M, Dayton, Ohio 45458. SCHEDULE: Monday - Friday from 8 AM - 5 PM and as needed/on call Field work + client home visits Monday-Friday as needed (with the ability to work from home 1-2 days per week when not required in the field). Willingness to provide occasional on-call and varied / flex hour coverage (late night / weekend emergency) is required as needed. Ability to report to our Jackson, OH Office in person as needed during State Surveys is required. RNs, LPNs, Home Health Aides, and Personal Care Assistants) while directing, coordinating, supervising, and implementing professional and supportive services to home health clients in a safe, cost-effective manner. Details: This is a full-time, salaried position requiring a blend of clinical, compliance, and business growth skills. Minimum availability requirements: This role will oversee clients and field staff across Southwest Ohio operations, serving Dayton, Cincinnati, Columbus, and surrounding areas.
