The Professional Staff Nurse/Case Manager at UPMC Muncy Place is responsible for delivering comprehensive nursing care and coordinating with interdisciplinary teams, residents, families, and external partners to ensure high-quality outcomes and seamless transitions post-discharge. This role involves managing both short- and long-term residents, supporting care planning, leading Crisis Intervention training, and promoting wellness through chronic condition management and education. The Case Manager collaborates with the Quality Improvement Team to monitor and reduce Emergency Department visits and hospitalizations, identifies educational needs, and provides guidance to frontline staff—all while applying age-appropriate care principles and maintaining a resident-centered approach. The ideal candidate will have the following: 2 years of RN experience, 5 years working in a Long Term Care facility, and a BSN. Schedule: Full-time, 40 hours/week (Day Shift, Monday–Friday) Holiday Rotation: 1-2 holiday shifts per year Weekend Rotation: Minimum 1 weekend shift every 4 weeks Sign-On Bonus $15,000 for new graduates (2-year commitment) $20,000 for experienced nurses (2-year commitment)
