Requirements

• Bachelor's or Master's degree in Social Work, or as required by state regulations.
• Minimum of 2 years of social work or case management experience in an inpatient setting highly preferred.
• Strong verbal and written communication skills in English; additional languages preferred.
• Proficiency in Microsoft Office applications required.

Nice To Haves

• Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN).
• Familiarity with CMS documentation standards and discharge planning is preferred.
• Experience in acute or rehabilitation hospital preferred.

Responsibilities

• Supports coordination of care, discharge planning, and communication with caregivers and the healthcare team.
• Collaborates across disciplines to ensure the development and implementation of individualized care plans that meet patient needs and regulatory standards.
• Demonstrates ability to coordinate patient care in an interdisciplinary setting.
