Requirements

  • Must be 18 years of age; must have a high school diploma or GED.
  • One year of experience in the field of intellectual/developmental disabilities, or successful completion of training in the Missouri Quality Outcomes approved by the Division of DD regional office.
  • Reliable means of communication required.
  • Must possess the ability to read and understand written materials.
  • Must possess skills to write clearly and legibly.

Responsibilities

  • Report to and work with assigned clients receiving non-residential services according to monthly units authorized.
  • Read and understand the content of each Individual Support Plan (ISP) and its addendums for clients on the caseload.
  • Provide support as specified in the ISP, including attending medical appointments, recreational activities, and performing personal hygiene tasks.
  • Deliver training in personal hygiene, money management, safety, wellness, household skills, meal preparation, and medication management.
  • Document progress on goals and objectives stated in the ISP.
  • Complete required documentation in Therap at the end of each shift, signed with first name, last name, and title.
  • Ensure all forms and needed paperwork are maintained in the client record.
  • Assist with billing information to meet accounting timelines and perform other duties as assigned by the Regional Director.
