We deliver high-quality, gender-affirming, trauma-informed medical, mental health, pharmacy, and supportive services to thousands of patients across the state and beyond. Our work advances LGBTQ+ health equity through care delivery, advocacy, education, research, and community partnership. The Role You will deliver evidence-based, patient-centered primary care to a diverse and cross-sectional patient population, including transgender and gender-diverse patients and people living with or at risk for HIV. Requirements

  • Strong primary care foundation with interest or experience in LGBTQ+ health and HIV care
  • Commitment to evidence-based, trauma-informed, and equity-driven practice
  • Ability to work efficiently in a collaborative, team-based care model
  • Comfort caring for patients with complex medical and social needs

Responsibilities

  • Provide comprehensive primary care across the lifespan
  • Deliver gender-affirming care, HIV prevention and treatment, and comprehensive sexual health services
  • Conduct routine follow-ups, annual physicals, urgent/sick visits, telehealth visits, and office-based procedures
  • Screen for and address preventive health, behavioral health, substance use, and social determinants of health
  • Manage patient care plans, including diagnostics, medications, referrals, and EMR messaging
  • Educate patients and care partners using trauma-informed, affirming approaches
  • Complete clinical documentation and close charts within organizational standards
  • Participate in panel management, population health initiatives, and quality improvement efforts
  • Collaborate closely with pharmacists, social workers, behavioral health clinicians, RNs, and APPs
  • Provide supervision, consultation, and clinical support to APPs and clinical staff as appropriate
  • Participate in peer review, case conferences, and ongoing clinical education
  • Evaluate and prepare letters of support for gender-affirming surgeries, when indicated
  • Participate in shared on-call responsibilities (details discussed during interview process)
