About The Position

Are you a patient-centered healthcare professional committed to delivering high-quality, respectful care to a diverse patient population? Callen-Lorde is seeking a Primary Care Provider (NP/MD/DO) who specializes in HIV care to join our collaborative, multidisciplinary team. As a PCP at Callen-Lorde, you key responsibilities will include: Clinical Care Conduct patient screenings, examinations, diagnoses, and treatments across the lifespan. Provide care that is respectful, compassionate, unbiased, and consistent with evidence-based guidelines. Deliver care in scheduled 20-40 minute appointments and ensure timely follow-up on patient needs, test results, and referrals. Document all care promptly in the electronic health record (EPIC). Provide primary care services Collaboration & Coordination Make appropriate internal and external referrals. Work effectively within a team-based care model, collaborating with other healthcare professionals to ensure coordinated patient care. Quality & Professional Development Participate in quality improvement initiatives and case conferences. Engage in ongoing professional development opportunities, including continuing medical education.

Requirements

  • New York State Certification/Licensure (MD, DO, or NP)
  • Current NYS Registration and DEA Controlled Substances Registration Certificate
  • Current CPR Certification
  • Board certified or eligible in Family Medicine, Internal Medicine, or Infectious Disease
  • Ability to work effectively with individuals from a wide range of backgrounds
  • Commitment to accessible, patient-centered primary care
  • Strong collaboration skills in a team-based model

Nice To Haves

  • Experience in community-based primary care preferred
