As a Primary Care Physician, you will guide the medical care of PACE participants through comprehensive assessments, individualized care planning, and ongoing clinical leadership within the Interdisciplinary Team.

Requirements

  • MD or DO from an accredited program
  • Board Certification in Internal Medicine or Family Medicine
  • Active, unrestricted Michigan medical license
  • CPR certification (or willingness to obtain)

Nice To Haves

  • Geriatrics and/or palliative care experience or certification
  • Experience caring for medically complex or frail older adults

Responsibilities

  • Perform initial and ongoing comprehensive health assessments
  • Lead clinical decision-making within the interdisciplinary care team
  • Develop and oversee personalized care plans aligned with participant goals
  • Partner with families to support informed, compassionate care decisions
  • Manage chronic conditions with a proactive, preventative approach
  • Collaborate with specialists and oversee care transitions, including skilled nursing stays
  • Work closely with the Medical Director and Advanced Practice Providers to ensure coordinated, high-quality care
