About The Position

Sanger Place MHRC (Mental Health Rehabilitation Center) is a 15-bed sub-acute, psychiatric care facility for adults ages 18-59 with serious mental illness and developmental disability. Primary Care Physician - Part-time 16 hours per week, 2 days Mon-Sun 8am-5pm onsite (Flexible on days, must be open to on call for emergencies other days of the week 5pm-8am) Compensation Employee Expected starting wage range is $140- $174 / Hourly Contractor Expected starting wage range is $181 - $227 / Hourly Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements. POSITION SUMMARY The Primary Care Physician is primarily responsible for providing medical services for members. In addition, the Primary Care Physician may be directed by the Chief Professional Practice Officer to perform other duties in accordance with funding agencies requirements, contractual agreements, and policies, procedures, and protocols established by Telecare Corporation.

Requirements

  • Graduate from an accredited medical school
  • Must have a valid license to practice medicine in the designated state
  • Professional clinical experience in specific area of practice, i.e., "Family Practice, Internal Medicine, etc."
  • Active DEA license
  • Must be at least 18 years of age
  • Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment
  • All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. If the position requires driving, valid driver license, a motor vehicle clearance and proof of auto insurance is required at time of employment and must be maintained throughout employment. Additional regulatory, contractual or local requirements may apply

Responsibilities

  • Demonstrate the Telecare mission, purpose, values and beliefs in everyday language and contact with the internal and external stakeholders
  • Assume quality of medical care for all patients.
  • Obtains appropriate patient history, performs physical evaluation, orders appropriate diagnostic tests and follows-up with interpretation, formulates assessment and initiates treatment plan with appropriate follow up appointments.
  • Reviews incoming reports (e.g. lab, x-ray, EKG) signs, dates and follows up with patients in a timely manner.
  • Ensures documentation is entered in a complete, timely and orderly manner in the patient's medical records.
  • Consults and refers with/to appropriate Specialty Physicians.
  • Adheres to the highest standards of medical practice, ethics, and professionalism at all times.
  • Assist in establishing medical policies, quality improvement procedures, evaluating current practices, policies and procedures.
  • Recommend changes in all clinical areas, and/or in general areas to improve the patient flow, medical records, billing practices, and appointment practices.
  • Serve as a member of the interdisciplinary team providing care to serious mentally ill adults.
  • Responsible for the directing and supervising the clinical staff, as appropriate.
  • Other duties as required by the customer to carry out contractual agreements and/or as established by Telecare Corporation
  • Must become familiar with the Federal and State rules, regulations, and practices.
  • Become familiar with Federal and State Health care program requirements and assist in carrying out these programs.
  • Assist in developing the whole health care plan and review of the program requirements and recommend changes when necessary to carry out the whole health plan as required.

  • Promotes and practices medicine consistent with Telecare's Recovery-Centered Clinical System philosophy.

  • Educate members served and families about the benefits and adverse effects of prescribed treatment options
  • Monitor/review progress in treatment in collaboration with the multidisciplinary treatment team. This process may include review of relevant laboratory or other diagnostic tests, communication with multidisciplinary team members, review of daily documentation, and ongoing members served interviews
  • Make necessary modifications to prescribed treatment regimen based on regular monitoring and review of progress
  • Make appropriate referrals to other healthcare and community resources as indicated
  • Provide necessary prescriptions to continue medications started at the program
  • Prepare and maintain documentation, records, and reports in accordance with established rules, regulations, and program guidelines
  • Perform duties within Scope of Practice in accordance with professional standards, established clinical protocols, and related Federal, State, and County regulatory requirements
  • Engages in ongoing education, supervision, and training as required. Be well informed on current best practice standards and guidelines in primary care.
  • Participates in Medical Quality Review, Peer Review and Utilization Review processes as required by the Facility, County, Telecare, and/or other relevant entity
  • Provide industry relevant tips for the Practice Site
  • Must assist with restraint of members served in the event of assaultive behavior and pass assault crisis/crisis prevention training
