About The Position
We are a patient-centered mobile care practice dedicated to delivering high-quality primary care directly to patients where they live. Whether in private homes, or assisted living communities, we bring compassionate, comprehensive medical care to those who need it most. Based in Livingston, NJ, we serve patients across Essex County and surrounding areas. We’re seeking a dedicated and empathetic Primary Care Physician to join our growing team. This role is ideal for a provider who values autonomy, flexibility, and meaningful patient relationships. You’ll provide routine and preventive care, manage chronic conditions, and coordinate with other healthcare professionals—all while working outside the traditional clinic setting.
Requirements
- MD or DO degree from an accredited medical school
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics
- Active New Jersey medical license and DEA registration
- Valid driver’s license and reliable transportation
- Strong interpersonal and communication skills
- Comfortable working independently and adapting to varied environments
Nice To Haves
- Experience in home-based care, geriatrics, or long-term care settings preferred
Responsibilities
- Conduct in-person primary care visits in patients’ homes and assisted living facilities
- Perform comprehensive assessments, diagnose and treat acute and chronic conditions
- Develop and manage individualized care plans
- Coordinate with specialists, caregivers, and facility staff to ensure continuity of care
- Document visits and maintain accurate medical records using EMR
- Provide patient education and support for families and caregivers
- Participate in quality improvement initiatives and case reviews
