Employer: Dedicated Senior Medical Centers of Florida, LLC Position Title: Primary Care Physician – multiple positions Position Description: Independently provides care for patients with acute and chronic illnesses in older adult patients. Manages patient care and outcomes and appropriately seeks consultation from specialists while staying involved in, and responsible for, the detailed care of the patient. Engages with hospitalist whenever their patients are in the hospital (regardless of whether the hospitalist works for ChenMed). Responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs. For patients unable to travel to the office—in hospital, SNF, LTC or homebound - engages with transitional care team and others including case managers, acute and transitional-care physicians, and other resources available. Leads Super Huddle (SH) and Transforming Care Meeting (TCM) weekly, and supports Center Manager/Center General Manager if the center clinical leader and/or market clinical leader is unavailable, based on guidance from Market Chief Medical Officer. May work at other Dedicated Senior Medical Center sites in Jacksonville, FL.

Requirements

  • Medical Degree or foreign equivalent
  • Board Certified/Board Eligible (BC/BE) in Family Medicine or Internal Medicine.
  • Must possess a Florida State Medical license and Drug Enforcement Administration (DEA) license.

Responsibilities

  • Independently provides care for patients with acute and chronic illnesses in older adult patients.
