About the Primary Care Physician / PCP Role: Provide comprehensive primary care, including preventive care and management of acute and chronic conditions. Develop personalized treatment plans and involve patients in decisions about their care. Provide lifestyle counseling to support long-term health and wellness. Coordinate care with specialists and support patients through referrals and follow-up. Monitor patient progress, review results, and ensure continuity of care. Document patient care accurately using the Electronic Health Record (EHR) system. Participate in quality improvement efforts and maintain evidence-based standards of care. Collaborate with a multidisciplinary team to provide integrated, patient-centered care.
