About Us Lac Courte Oreilles Community Health Center is a Patient-Centered, Medical Home Model providing comprehensive, integrated care for the tribal community, bringing medical, dental, vision, pharmacy, and wellness services together in one place. With a mission focused on improving access and health outcomes, the team works collaboratively to deliver coordinated, patient-centered care without financial barriers. About the Primary Care Physician / PCP Role: Provide comprehensive primary care, including preventive care and management of acute and chronic conditions. Develop personalized treatment plans and involve patients in decisions about their care. Provide lifestyle counseling to support long-term health and wellness. Coordinate care with specialists and support patients through referrals and follow-up. Monitor patient progress, review results, and ensure continuity of care. Document patient care accurately using the Electronic Health Record (EHR) system. Participate in quality improvement efforts and maintain evidence-based standards of care. Collaborate with a multidisciplinary team to provide integrated, patient-centered care.

Requirements

  • Qualified & Licensed: MD or DO from an accredited medical school with completion of a Family Medicine or Internal Medicine residency. Board certification or active pursuit of certification in a primary care specialty is required. Ability to obtain Wisconsin medical license, DEA license, and meet Medicare/Medicaid requirements.
  • Experienced Clinician: Strong diagnostic skills with the ability to manage both acute conditions and complex chronic illnesses, including behavioral health and substance use challenges.
  • Patient-centered: Builds trust with patients through clear communication, empathy, and a non-judgmental approach to care.
  • Relationship-driven: Invested in developing long-term patient relationships and making a meaningful impact in the community.
  • Organized & Efficient: Manages time effectively while maintaining high-quality, thorough patient care.
