About The Position

This physician-led organization is redefining primary care for older adults through a high-touch, value-based Medicare Advantage model. The practice model is built around patients, allowing physicians to truly care for them with significantly smaller patient panels, longer visit times, and unmatched care team support. This provides autonomy and resources to deliver high-quality care. The opportunity features ultra-low patient panels (approximately 400–450 patients), extended visit times (typically 30–60 minutes per patient), and a high-touch care model focused on accessibility and continuity. It emphasizes a proactive, prevention-first approach with daily patient engagement and includes a dedicated care team comprising MAs, Care Coordinators, and Care Facilitators. Physicians receive on-site leadership support and benefit from a strong physician mentorship culture within a mission-driven organization focused on underserved senior populations. The model is designed to prevent burnout by enabling daily patient follow-ups and deeper engagement, with integrated care teams handling administrative and coordination tasks, and data-driven insights supporting effective population health management.

Requirements

  • MD or DO
  • Board Certified/Eligible in Internal Medicine, Family Medicine, or Geriatrics
  • Active or eligible Texas medical license
  • Active DEA
  • Passion for serving seniors and underserved communities
  • Commitment to a value-based, outcomes-driven model of care
  • Strong interpersonal skills and a relationship-first mindset

Responsibilities

  • Provide comprehensive, relationship-driven care to Medicare Advantage patients
  • Serve as both a clinician and trusted health coach, guiding behavior change and preventive care
  • Maintain full accountability for patient outcomes, experience, and total cost of care
  • Conduct frequent touchpoints with patients, including same-day and walk-in availability
  • Coordinate care across specialists, hospitals, and post-acute settings
  • Lead and collaborate closely with your care team to ensure seamless patient support
  • Supervise and mentor Advanced Practice Providers (APPs)
