Job Summary We are seeking a Primary Care Nurse Practitioner with experience in Family or Internal Medicine to join our team in Muskego, WI! The ideal candidate will deliver broad-scope primary care services and coordinate health care delivery for a panel of patients assigned to the Nurse Practitioner’s care. The Solidaritus health direct primary care model provides Nurse Practitioners the gift of time to provide world-class care and to: Listen to, and care for, patients Develop, adjust, and support patient adherence to individualized health care action plans Prevent and manage chronic disease with support of interactive tools and processes Respond to patient’s urgent and episodic sick care needs Provide ongoing healthcare education for patients and families Coordinate overall delivery of care across all care settings Develop a trusting provider-patient relationship with individuals and families under their care, becoming a trusted provider and guide in navigating the healthcare system and making key healthcare decisions

Requirements

  • Valid and current nursing license and nurse practitioner certification from an accredited program
  • DEA license
  • Experienced in either Family or Internal Medicine
  • Commitment to patient-centered care and continuous improvement
  • Strong leadership and communication skills
  • Proficiency with electronic medical records
  • A spirit of collaboration and ability to coach patients toward healthy changes

Responsibilities

  • Deliver comprehensive primary care, including preventative and wellness care, chronic disease management, and urgent and episodic care
  • Conduct physical exams and develop Individualized Health Care Action Plans (HCAPs)
  • Provide health education and lifestyle coaching
  • Coordinate care across all settings and manage referrals
  • Order and interpret diagnostic tests
  • Perform primary care procedures (e.g. laceration repair, biopsies)
  • Document patient encounters accurately in EMR
  • Direct clinical support staff
  • Utilize technology for efficient patient management
  • Participate in continuous learning and improvement of care delivery
