Candidates with a background in Family Medicine or Internal Medicine–focused primary care will be considered. This is a full-time, permanent position providing comprehensive primary care services to adult and geriatric patients in an outpatient setting. Requirements
- Education: Master’s or Doctoral degree in Nursing from an accredited program
- Licensure: Active Connecticut APRN license (or eligibility)
- Certification: Board Certified as FNP or Adult/Gerontology NP
- Training Focus: Family Medicine or Internal Medicine–based primary care
- Experience: All experience levels welcome; new graduates encouraged to apply
Responsibilities
- Provide comprehensive primary care services, including preventive care, chronic disease management, and acute visits
- Diagnose, treat, and manage common medical conditions across the adult patient population
- Conduct annual physicals, wellness visits, and health risk assessments
- Order and interpret diagnostic tests and laboratory results
- Collaborate with physicians, specialists, and interdisciplinary care teams
- Maintain accurate and timely documentation in the EMR
- Participate in quality improvement initiatives and evidence-based care delivery
- Educate patients on disease prevention, treatment plans, and healthy lifestyle choices
