This is a full-time, permanent position providing comprehensive primary care services to adult and geriatric patients in an outpatient setting. Requirements

  • Education: Master’s or Doctoral degree in Nursing from an accredited program
  • Licensure: Active Connecticut APRN license (or eligibility)
  • Certification: Board Certified as FNP or Adult/Gerontology NP
  • Training Focus: Family Medicine or Internal Medicine–based primary care
  • Experience: All experience levels welcome; new graduates encouraged to apply

Responsibilities

  • Provide comprehensive primary care services, including preventive care, chronic disease management, and acute visits
  • Diagnose, treat, and manage common medical conditions across the adult patient population
  • Conduct annual physicals, wellness visits, and health risk assessments
  • Order and interpret diagnostic tests and laboratory results
  • Collaborate with physicians, specialists, and interdisciplinary care teams
  • Maintain accurate and timely documentation in the EMR
  • Participate in quality improvement initiatives and evidence-based care delivery
  • Educate patients on disease prevention, treatment plans, and healthy lifestyle choices
