Requirements

  • Current New Hampshire Nursing License (RN or LPN) required.
  • Experience in primary care or ambulatory health preferred.
  • Current BLS certification required.
  • Strong organizational, communication, and clinical assessment skills.
  • Compassionate, adaptable, and team-oriented mindset.

Responsibilities

  • Rotate among all primary care teams to provide hands-on and telephonic patient care.
  • Collaborate with providers, MAs, and multidisciplinary staff to coordinate care and ensure smooth transitions.
  • Perform clinical duties such as patient education, care coordination, medication review, and follow-up.
  • Manage test results, referrals, and care plans in collaboration with the clinical team.
  • Deliver compassionate, patient-centered care - every interaction, every day.
